We're looking for an experienced Registered Nurse leader to join our team as a Director of Patient Care In this role, you'll lead our hospice clinical team, support excellent patient care, and help create a culture where both patients and employees feel supported.

What You'll Do

Lead and oversee the hospice clinical team and patient care operations.
Ensure patients receive safe, compassionate, high-quality hospice care.
Supervise, coach, and support clinical staff.
Monitor staffing, productivity, and clinical performance.
Ensure patient care is provided according to individualized Plans of Care.
Assist with recruiting, interviewing, onboarding, and training clinical team members.
Identify opportunities for staff development and continuing education.
Oversee QAPI and utilization review activities.
Ensure compliance with hospice, Medicare, state, and federal regulations.
Monitor clinical documentation and patient care processes.
Work closely with the Administrator and leadership team on agency goals, policies, and operations.
Identify opportunities to improve patient care and clinical operations.
Build strong relationships with physicians, referral partners, patients, and families.
Serve as a clinical resource and leader for the hospice team.

What You'll Need

Current, active Registered Nurse (RN) license
2–3 years of recent RN experience
Hospice experience preferred
Previous management or clinical leadership experience preferred
Strong communication and interpersonal skills
Excellent clinical judgment and decision-making abilities
Ability to lead, coach, and develop a clinical team
Strong understanding of hospice regulations and standards of care
