Full Job Description

CBRF-Certified Caregiver and Medication Administrator PM Shift

Job Overview

We are seeking a dependable, compassionate, and experienced CBRF-Certified Caregiver and Medication Administrator to join our team on the PM shift.

In this role, you will administer medications, assist residents with activities of daily living, document services, and help maintain a safe, respectful, and supportive living environment. The ideal candidate understands that quality care involves more than completing tasks. It means building relationships, protecting resident dignity, recognizing changes in condition, and helping each resident maintain the highest possible level of independence.

Essential Responsibilities

Administer medications according to physician orders, the medication administration record, facility policies, and Wisconsin CBRF requirements.
Accurately document medication administration, refusals, omissions, effectiveness, side effects, and other medication-related concerns.
Immediately report medication errors, adverse reactions, changes in condition, and other health or safety concerns according to facility procedures.
Obtain and document vital signs when required by a resident’s service plan, physician order, or nursing direction.
Assist residents with bathing, grooming, dressing, toileting, continence care, oral hygiene, mobility, transfers, and other activities of daily living.
Provide respectful and person-centered care to residents living with dementia, Alzheimer’s disease, developmental disabilities, mental health conditions, or physical disabilities.
Follow each resident’s assessment, individual service plan, behavior support plan, dietary requirements, and established safety precautions.
Perform nursing-related or specialized care tasks only when properly trained, determined competent, and delegated or directed by the facility’s registered nurse.
Assist with meals, hydration, snacks, activities, housekeeping, laundry, and other household responsibilities as assigned.
Complete accurate and timely documentation in the facility’s electronic health record and electronic medication administration record systems.
Maintain resident confidentiality and comply with HIPAA requirements.
Follow standard precautions, infection-control procedures, emergency procedures, fire-safety requirements, and resident-rights protections.
Communicate professionally with residents, families, coworkers, supervisors, nurses, healthcare providers, pharmacies, and other members of the care team.
Participate in shift-to-shift communication and provide complete reports regarding resident needs, behaviors, incidents, medication concerns, and changes in condition.
Respond appropriately to emergencies and provide first aid or choking assistance within the employee’s level of training.

Required Qualifications

Current Wisconsin DHS-approved CBRF Medication Administration and Management training.
Current Wisconsin DHS-approved Standard Precautions training.
Current Wisconsin DHS-approved First Aid and Choking training.
Current Wisconsin DHS-approved Fire Safety training.
Ability to provide documentation of completed CBRF training through the Wisconsin Community-Based Care and Treatment Training Registry.
Ability to accurately administer medications and complete medication documentation.
Ability to read, understand, and follow physician orders, medication records, service plans, and facility policies.
Strong communication, observation, organization, and documentation skills.
Ability to maintain confidentiality and protect resident information.
Ability to safely assist residents with mobility, transfers, repositioning, and personal care using proper techniques and required equipment.
Ability to successfully complete all required caregiver background checks and employment screenings.
Availability to work PM shifts, including assigned weekends and holidays.

Preferred Qualifications

Previous experience working in a CBRF, assisted living facility, memory-care community, adult family home, nursing home, hospital, or home-care setting.
Experience administering medications using an electronic medication administration record system.
Experience supporting individuals living with dementia, developmental disabilities, mental health conditions, hospice needs, or complex personal-care needs.
CPR certification or willingness to obtain certification after hire.
