Full Job Description

Purpose and Objectives: Supervise case managers working directly with older adults age 60 and older in order to identify client’s needs and service preferences, and work with client (and family, when appropriate) to develop, implement and oversee a care plan to meet mutually agreed upon goals.

Organizational Values: The Community Agency for Senior Citizens (CASC) assists Staten Islanders age 60 and older to live as independently as possible in the community. We do this by working in hand-in-hand with older adults, respecting their right to make decisions for themselves, honoring their life experiences and heritage, and recognizing the importance of friends, relatives and community in creating supportive environments for people as they age.

Position Qualifications:

§ Master’s Degree in Social Work (or related Master’s level degree)

§ Minimum 3 years full-time experience in social services or related field At least 2 years supervisory experience

§ Proven leadership experience

§ Crisis management skills

§ Excellent communication skills

§ Experience in the field of aging

Requisite Skills and Capabilities:

§ Detail oriented: ensure that all policies, procedures and requirements for program are met during required timeframes

§ Leadership: ability to identify strengths and weaknesses in staff and provide supervision, education and guidance to improve job performance, and to manage team during crises and other stressful situations

§ Team Leader: ability to create collaborative work environment and lead staff to articulate and work toward common goals

§ Understands diversity: accommodates linguistic and cultural differences; works well with colleagues and clients with diverse ethnic and cultural backgrounds and can guide staff in meeting these goals

§ Self-motivated: Able to work independently, work well under pressure and handle multiple priorities simultaneously

§ Analytical: reviews options and considers implications prior to making decisions, develops systematic and logical procedures, presents clear and logical solutions

§ Ability to represent CASC at internal meetings and community events including presentations, committees, coalitions and meetings with key stakeholders

§ Creative problem-solving during crisis situations, to motivate team members, and contribute to Continuous Quality Improvement Efforts

Responsibilities:

§ Responsible for all aspects of case management program

§ Supervise Assistant Program Director, four Supervisors and one Case Aide, subject to change per needs of program

§ Assigns cases in consultation with supervisors

§ Conduct monthly team meetings and other staff meetings and supervisory sessions as required

§ Provide guidance and training to ensure employees understand the company’s workflows and NYC Aging procedures; arrange additional trainings as needed

§ Ensure that all NYC Aging procedures and requirements are fulfilled, including maintaining NYC Aging required average caseloads

§ Oversight of VIVE data management system, ensuring timely and accurate data input and generation and submission of required statistical and programmatic reports to NYC Aging and, internally, to CASC Chief Program Officer, President/CEO, Board of Directors and other oversight bodies as required

§ With CASC fiscal department, responsible for monitoring expenses to ensure program remains within budget

§ Maintain familiarity with all relevant NYC Aging standards, including updates to standards and procedures

§ Represent CASC at community events, committees and coalitions

§ Assist with other program activities as assigned

Key Results:

§ Case management program operates effectively and efficiently

§ Detailed program record keeping leads to timely submission of reports as required by NYC Aging and CASC

§ Continuous Quality Improvement goals achieved

§ Case management clients receive high quality, individualized services and support to help them remain safely in their homes and communities

§ Effective implementation of CASC procedures and guidelines

Supervision and Education:

§ Supervised by Chief Program Officer

§ Participate in team meetings

§ Participate in NYC Aging and CASC training and education sessions

§ Participate in local educational and informational sessions and conferences

§ Annual personnel review

Salary and Benefits:

§ $85,000-$88,000 per year
