Job Summary

Join our dedicated healthcare team as a Certified Nursing Assistant (CNA) / Home Health Aide (HHA), where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance with activities of daily living, monitor patient health, and support overall well-being in home and community settings.

Your energetic approach and commitment to quality care will help promote independence, comfort, and dignity for individuals of all ages and needs, including those with developmental disabilities, dementia, or recovering from illness. This paid position offers an engaging environment where your skills in patient observation, medical documentation, and caregiving are valued and expanded.

Responsibilities

Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
Monitor vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately
Support medication administration following proper protocols while maintaining HIPAA compliance to ensure privacy and safety
Document patient care activities meticulously using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for seamless medical documentation
Provide specialized dementia and Alzheimer’s care, including memory care strategies and behavioral support techniques
Assist with medical procedures such as catheter care and hospice care support as directed by healthcare professionals
Perform light housekeeping duties like cleaning, laundry, meal preparation, and ensuring a safe environment for clients in their homes or assisted living facilities
Observe patients closely for any changes in condition or behavior related to disabilities or chronic illnesses and communicate findings promptly to the healthcare team

Experience

Prior experience working in nursing homes, assisted living facilities, or home health environments is highly preferred
Demonstrated ability to assist with Activities of Daily Living (ADLs) such as feeding, toileting, and mobility support
Experience working with individuals with developmental disabilities or dementia enhances your ability to provide compassionate care
Knowledge of medical terminology and familiarity with medical records management is advantageous
Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or catheter care are a plus
Proven ability to operate EMR/EHR systems efficiently while maintaining confidentiality under HIPAA guidelines
Long-term care experience or live-in employment background demonstrates your dedication to ongoing patient support
