Job Description

The incumbent of this position is responsible for supporting families with low incomes achieve upward economic mobility utilizing the whole family approach. The incumbent will help the families overcome barriers to stability and financial independence through effective family-centered coaching and coordinated service delivery through programs available within CAPHR and with connections to community partner agencies.

SUPERVISION:

Received: Works under the direct supervision of the Family Centered Services Manger and the general supervision of the Innovations and Strategy Director.

Exercised: None.

RATE: $25.43/hr

JOB RESPONSIBILITIES:

Using family-centered coaching strategies, the Opportunity Navigator partners with families to identify strengths, assess challenges, set meaningful goals, and leverage resources both within CAPHR and with other partner agencies.
Work collaboratively with families to support access to resources that promote long-term stability. This includes assistance across a broad range of needs such as economic security, stable housing, education, employment, childcare, healthcare and transportation.
Monitor families’ progress through timely, ongoing engagement to ensure goals are being met and to make course corrections, as needed.
Continually remain up to date about all CAPHR programs available to individuals and families to ensure access to wraparound services.
Maintain up-to-date knowledge of community resources available to individuals and families to address the needs beyond the scope of CAPHR programs; maintain connections with community partners to ensure warm hand offs can be made.
A core component of this role includes delivering CAPHR’s “Your Money in Action” (YMIA) financial literacy training and providing ongoing one-on-one financial counseling to help families develop practical financial skills and achieve short-, medium-, and long-term financial goals.
On an as-needed basis, support CAPHR’s Emergency Solutions Grant (ESG) and Housing Stabilization Services (HSS) homelessness prevention programs in screening and enrolling eligible participants.
On an as needed basis, support CAPHR’s Volunteer Income Tax Assistance program team in enrolling eligible participants for the yearly income tax returns filing.
Attend training and remain up to date on family-centered coaching and whole family practices.
Attend community events to promote awareness about CAPHR’s programs in general and the YMIA program in particular. This can require making presentations to groups of community members and partner agencies staff.
Maintain detailed client records and maintain up to date information in appropriate database system/s.
Maintain professional boundaries with all current, past, and prospective clients, and maintain the confidentiality of clients and staff, in accordance with CAPHR policies and procedures.
Abide by all CAPHR Safety policies and procedures.
Present professional and positive image as a representative of CAPHR.
Travel to CAPHR offices and other locations within the State for case management meetings and networking events.
Perform other related duties as required.

REQUIRED SKILLS AND ABILITIES:

Strong community relations skills, including interpersonal, written and oral communication.
Knowledge of the barriers facing the low-income population and the relevant field-supported interventions, including Trauma Informed Care.
Natural empathy towards the needs of low-income individuals and the challenges they face.
Strong knowledge of local, state, and federal human service systems and resources
Bilingual (English and Spanish) proficiency will be an advantage

EDUCATION AND/OR EXPERIENCE:

Bachelor’s degree in human services or related field preferred
Training and/or certification in financial coaching will be an advantage
Experience working with families with low incomes
Knowledge of social service systems and mainstream benefits for families with low incomes
