Physical Therapist

Job Summary

Our outpatient physical therapy clinic is seeking a licensed Physical Therapist to join our team. The ideal candidate is patient‑focused, skilled in neurologic, geriatric, orthopedic and musculoskeletal rehabilitation, and comfortable working in a dynamic outpatient environment. You’ll help patients improve mobility, reduce pain, and return to their daily activities through evidence‑based care and personalized treatment plans.

Key Duties & Responsibilities

Patient Evaluation & Assessment

Perform comprehensive initial evaluations, including functional assessments, range‑of‑motion testing, strength testing, and special tests.
Review patient medical history, referral information, and relevant clinical details.
Identify impairments, functional limitations, and patient‑specific goals.

Treatment Planning

Develop individualized treatment plans based on clinical findings and patient goals.
Establish clear short‑term and long‑term functional objectives.
Adjust treatment plans as patients progress or needs change.

Therapeutic Intervention

Provide evidence‑based interventions including therapeutic exercise, manual therapy, neuromuscular re‑education, gait training, and functional strengthening.
Utilize modalities such as heat/cold therapy, electrical stimulation, ultrasound, and traction when appropriate.
Teach and reinforce home exercise programs to support continued progress.

Patient Communication & Education

Educate patients on their condition, expected outcomes, and self‑management strategies.
Maintain open, supportive communication to encourage engagement and adherence.
Communicate effectively with referring providers, clinic staff, case managers, interdisciplinary team members, and third party payers
Promote patient engagement, adherence, and satisfaction throughout the episode of care.

Documentation & Professional Standards

Maintain accurate, timely clinical documentation in accordance with clinic policies.
Record evaluations, daily treatment notes, progress updates, and discharge summaries.
Uphold professional standards, ethical practice, and state regulatory requirements.

Clinic Operations & Team Collaboration

Work closely with PTAs, aides, and administrative staff to support smooth clinic flow.
Participate in team meetings, continuing education, and quality‑improvement initiatives.
Contribute to a positive, professional, and patient‑centered clinic environment.

Supervision, Mentorship & Academic Collaboration

Supervise and mentor physical therapy and physical therapy assistant student interns, providing guidance on clinical skills, professional development, and patient care standards.
Oversee student documentation, treatment planning, and patient interactions to ensure safe, effective, and ethical practice.
Collaborate with physical therapy and physical therapy assistant schools’ clinical education departments to coordinate student placements, communicate performance updates, and support academic program requirements.
Participate in student evaluations and contribute to a positive learning environment.

Qualifications

Doctor of Physical Therapy (DPT) degree.
Current New York State Physical Therapy License and Registration
Clinical Specialist Certification, or any relevant practice area certification is highly preferred
Strong communication, clinical reasoning, and time‑management skills.
CPR/BLS certification preferred.

Job Type: Full-time

Pay: $90,000.00 - $115,000.00 per year
