Full Job Description

Overview of Position:

Seeking an experiencing case manager located in Central or Western Kansas.

The Case Manager is responsible for providing short-term, housing-focused case management services to families participating in the Housing Support Program for Families across 101 counties in Kansas. This position is primarily remote, with some travel required.

This role is critical to supporting families who are experiencing homelessness, at risk of homelessness, or housing instability by helping them access housing assistance, develop individualized housing stability plans, and connect to community-based resources that support long-term housing stability.

Working closely with landlord engagement specialists, regional partners, and the Coordinated Entry System, the Case Manager supports homelessness prevention, diversion, and rapid re-housing efforts through trauma-informed, person-centered, and Housing First-aligned services.

Requirements and Qualifications:

· Minimum of 2 years of experience in rapid re-housing or homelessness prevention case management.

· Preferred experience with HMIS, CES, or the Continuum of Care (CoC).

· Preferred experience working with families, children, or households experiencing homelessness or housing instability in rural or multi-county service areas.

· Candidates with lived experience of homelessness, housing instability, or related systems are encouraged to apply.

· Must have a valid license and insured vehicle.

· Bachelor’s degree or an equivalent combination of applicable training and experience is required. Preferred fields include Social Work, Human Services, Public Health, Family Studies, or a related field.

· Preference to individuals who live in the KS BoS CoC Service Area

Responsibilities Include (including but not limited to the following:)

Conduct screenings and eligibility assessments for families referred to the Housing Support Program.
Collect and review required documentation related to household composition, income, housing status, and program eligibility.
Complete housing acuity assessments to determine the level of support needed.
Provide short-term, trauma-informed case management services to families experiencing or at risk of homelessness.
Develop individualized 12-month housing stability plans focused on housing, income, childcare, healthcare, and other needs impacting long-term stability.
Support families with goal setting, problem-solving, tenancy skills, budgeting, lease education, and connection to ongoing supports.
Conduct home or virtual visits, as needed.
Connect families to community resources such as employment, childcare, healthcare, behavioral health, transportation, legal assistance, benefits, and financial coaching.
Coordinate with landlord engagement specialists to identify housing opportunities, address barriers, and support successful lease-up.
Help families navigate applications, move-in planning, landlord communication, and housing stabilization.
Support families in accessing allowable financial assistance, including rent, utilities, deposits, moving costs, hotel/motel assistance, or other eligible housing-related supports.
Maintain accurate and timely documentation of client contacts, services, referrals, financial assistance, housing plans, and outcomes in HMIS.
Assist with monthly reporting, reimbursement documentation, performance tracking, and program evaluation.
Follow program policies and procedures aligned with grant requirements, ESG Rapid Re-Housing and Homelessness Prevention practices, Housing First, and low-barrier service delivery.
Participate in training and provide feedback on service gaps, barriers, and opportunities for program improvement.

Knowledge, Skills & Abilities

· Strong written and verbal communication skills, including the ability to document services clearly and professionally.

· Strong organizational skills with the ability to manage multiple cases, deadlines, and documentation requirements.

· Ability to establish professional and productive relationships with clients, landlords, service providers, community partners, and internal teams.

· Proficient in Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.

· Ability to learn and use HMIS or other required data systems.

Pay: From $24.00 per hour
