Full Job Description

Community Resource Center for the Developmentally Disabled (CRC) is committed to enable and support individuals with intellectual and developmental disabilities in their efforts to live and participate in the community as a natural right, in order to provide them the opportunities of inclusion, individualization, independence and productivity.

CRC is a not-for-profit community based organization which exists to establish, maintain and operate Intermediate Care Facilities, Individual Residential Alternatives, and to provide programs of care, service, habilitation, rehabilitation, and social/recreational activities geared to the functional capacity and developmental needs of each consumer in our care, so that they may be helped to experience living independently, making meaningful choices, enjoy personal relationships, communicate freely and receive effective services.

SUMMARY

Under general supervision, is responsible for the development and implementation of the Individualized Service Plans for individuals and their advocates in accordance with the regulations and requirements of the applicable governmental and other regulatory agencies within a residential setting. Provides case management services to assigned individuals and determines individual aptitudes, interests, and preferences. Identifies appropriate service providers and links individuals with providers. Coordinates services and monitors individual’s satisfaction with the services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The duties and responsibilities of the Case Manager will include, but are not limited to the following:

Provides social services to each resident directed toward maximizing their social functioning, enhancing coping capacity of their family, asserting, and safeguarding their human and civil rights and those of their family and fostering their human dignity and personal worth.
Develops and monitors individual treatment plan in accordance with regulatory and agency policy.
Participates in interdisciplinary planning. Interprets, documents, and follows up on all recommendations of interdisciplinary team meetings. Chairs all meetings of the interdisciplinary team.
Includes channels of communications and consultation with residence staff and professional staff to interpret and explain residence’s psychological needs as they progress through habilitation.
Develops goals and activities generated by interdisciplinary team. Produces an active treatment plan schedule for each individual served.
Documents comprehensive functional assessment and review of plan of care as needed.
Ensures proper data collection through observation and in-service training.
Assists in all activities of daily living and participates in activities outside of the residence i.e., shopping, trips, swimming pool, etc.
Supervises Residential Managers in implementation of active treatment plans.
Makes regular and meaningful family, resident, and agency-wide contacts.
Trains and instructs Residential Managers on individual’s specific goal plans, relevant data collection, daily routine and general procedures and practices of residence.
Maintains contacts with government and not-for-profit social service agencies to access resources and/or advocate for services.
Performs individual evaluations to determine whether and when alternate placement is needed.
Handles family outreach and in-service training.
Ensures that all benefits for which residents may be eligible for are applied for in an expeditious manner and to effect transfer of any prior accumulated funds to agency.
Provides systematic follow-up to assure referral to appropriate community agencies upon discharge.
Works regularly with, develops, or is responsible for maintaining security of confidential information such as records of person receiving services which requires regular use of discretion.
Coordinates admissions to program including preparation of referral material, initial screening of applicants, contact with families, and other referral services.
Ensures all required adaptive equipment is secured and maintained in good working order.
Maintains up to date files on applicants.
Advocates for residents to ensure that all their rights are being fulfilled.
Writes monthly case notes on all client’s interpersonal, social, workshop progress and family contacts.
Serves as a liaison to Day Programs, Service Coordination and Clinics as needed.
Keeps abreast of changes in field.
Performs other related duties as requested.

QUALIFICATIONS

Good interpersonal skills are required to interact effectively with employees, co- workers, residents, families and outside contacts.
Proficient in the use of a personal computer. And knowledge of appropriate software.
Due to sensitive nature of the position, must be able to maintain the strictest confidentiality.
Ability to work independently and handle multiple tasks.
Excellent organizational skills.
Able to work flexible hours.
Demonstrate sound judgment and the capacity to respond to unusual situations.

EDUCATION and/or EXPERIENCE

Bachelor’s Degree in the Human Services field.
At least one year of prior experience working with people with developmental disabilities.

LANGUAGE SKILLS

Ability to read, write, speak, and understand English.
Ability to read, analyze, and interpret reports and documents.
Ability to respond to common inquiries, concerns or complaints from employees and customers.
Excellent written and verbal communication skills required.

REASONING ABILITY

Ability to define problems, collect data, establish facts, and draw valid conclusions.
Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables.
Ability to resolve conflicts and mediate stressful situations.

WORK ENVIRONMENT

While performing the duties of this job, the employee is exposed to a normal residential/office setting with little or no discomfort due to noise or temperature. Some walking, bending, and standing required in order to photocopy, access files and so forth; work may be performed under pressure and may be, at times, mentally fatiguing; ability to sit for long periods of time necessary.
