Qualifications

Bachelor’s degree in Human Services or related field required; MSW preferred.
Experience working with NYC housing voucher programs, subsidies, and supportive housing models.
Demonstrated experience negotiating or coordinating with landlords and property management entities serving special needs populations.
Knowledge of applicable NYC and NYS housing, disability, and fair housing laws and regulations.
Experience using housing and care management databases such as HOME, CARES, CAPS, Housing Connect, and related systems.
Strong organizational, documentation, and data-tracking skills.

Job Summary
Coordinated Behavioral Care (CBC) is contracted with NYC Health + Hospitals Housing for Health to support the placement and retention of approximately 400 individuals annually into permanent supportive or affordable housing. Under the supervision of the Housing Retention Supervisor, the HLPS Housing Retention Specialist maintains a caseload and provides oversight, coordination, and technical support to HLPS partner agencies to strengthen housing stability and prevent returns to homelessness. The role ensures consistent engagement with housed clients, strong collaboration with stakeholders, and continuous quality improvement across housing retention activities.

Duties

Team & Collaborative Tasks

Serve as a primary point of contact and coordinator with Health + Hospitals Housing for Health staff, housing providers, managing agents, and landlords.
Collaborate with partner case management staff to resolve housing-related crises or conflicts with landlords, building owners, or property management.
Coordinate with partner agencies to support client transitions into housing and access to community-based supports as needed.
Ensure eligible clients are enrolled in a Medicaid Health Home Care Management program.
Provide referrals to medical, behavioral health, and social service providers appropriate to client needs.
Maintain ongoing communication with partners regarding housing retention concerns, emergencies, and stabilization efforts.
Conduct follow-up calls and home visits to assess housing conditions, client stability, and emerging risks; communicate concerns to partner case management staff.
Support aftercare and alumni activities, including workshops and educational sessions that strengthen long-term housing stability.
Attend assigned HLPS and program meetings.

Quality and Compliance

Document all client, partner, and collateral interactions in the Health + Hospitals General Housing Information Tracker (GHIT).
Review housed client records to ensure documentation is accurate, complete, and timely, including supportive services and housing status.
Ensure documentation reflects apartment conditions, inspections, readiness, and client engagement as required.
Establish, implement, and report corrective action plans to address gaps in required partner contact or documentation.

Reporting and Documentation

Review and maintain client records across GHIT, CAPS, PSYCKES, and other relevant data systems.
Prepare and document housing status changes, findings, and service activities.
Track, monitor, and report housing placement and retention updates in GHIT.
Maintain current housed-client lists to verify partner aftercare contacts and confirm housing status.

External Partnerships

Engage as an active team member with Health + Hospitals Housing for Health staff.
Participate in market housing opportunity meetings and follow up on assigned action items.
Attend supportive housing association meetings and other relevant forums to remain informed of housing system developments.

Other

Participate in professional development activities, including trainings, seminars, webinars, and conferences, as required.
Assist with intake coordination and coverage as needed.
Perform other related duties as assigned by the Program Director or designee
