Join our dynamic healthcare team as a Service Coordinator for the NHTD (Nursing Home Transition and Diversion) Program, where your expertise will directly impact patient care and service delivery. In this vital role, you will coordinate and manage comprehensive case services for individuals transitioning from hospital or long-term care settings, ensuring seamless support tailored to each patient's unique needs. Your energetic approach will help facilitate efficient discharge planning, resource allocation, and interdisciplinary communication, fostering positive health outcomes and patient satisfaction. This position offers an exciting opportunity to utilize your critical care, hospital experience, and case management skills in a fast-paced, rewarding environment committed to excellence in patient-centered care.

Position Summary

We are seeking a BILINGUAL RUSSIAN SPEAKING person who is compassionate, organized, and motivated Service Coordinator to join our Nursing Home Transition and Diversion (NHTD) Waiver Program team. The Service Coordinator will assist individuals with physical disabilities and seniors in accessing community-based services that support independent living and prevent unnecessary institutionalization.

This role involves conducting assessments, developing service plans, coordinating community resources, advocating for participants, and maintaining compliance with New York State Department of Health (NYSDOH) regulations and NHTD Waiver standards.

Key Responsibilities

Serve as the primary point of contact for patients, families, healthcare providers, and community agencies to coordinate services effectively.
Conduct initial and ongoing assessments of participant needs, strengths, goals, and eligibility
Develop, implement, and monitor individualized Service Plans (ISPs) and Revised Service Plan (RSP) in collaboration with participants and interdisciplinary teams
Coordinate services and referrals including housing, home care, medical care, behavioral health, transportation, and community supports
Maintain regular contact with participants through home visits, phone calls, and community meetings
Advocate for participant rights, independence, and informed choice
Monitor participant health and welfare to ensure services are effective and appropriate
Maintain accurate, timely, and compliant documentation in accordance with NYSDOH and agency standards
Participate in interdisciplinary meetings, trainings, supervision, and quality assurance activities
Assist participants transitioning from nursing homes or other institutional settings into the community
Maintain knowledge of Medicaid Waiver services, community resources, and eligibility requirements

Qualifications

Service Coordinator staff qualifications are as follows:

Persons self-employed or employed as Service Coordinators must be a/an:

An individual meeting any of the qualifications above must also have, at a minimum, one (1) year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.
Licensed Master Social Worker (Licensed by the NYS Education Department);
Licensed Clinical Social Worker (Licensed by the NYS Education Department);
Individual with a Doctorate or Master of Social Work;
Individual with a Doctorate or Master of Psychology;
Individual with a Master of Gerontology;
Physical Therapist (Licensed by the NYS Education Department);
Registered Professional Nurse (Licensed by the NYS Education Department);
Certified Teacher of Students with Disabilities (Certified by the NYS Education Department);
Certified Rehabilitation Counselor (Certified by the Commission of Rehabilitation Counselor Certification);
Licensed Speech Pathologist (Licensed by the NYS Education Department);
Occupational Therapist (Licensed by the NYS Education Department).

Persons self-employed or employed as Service Coordinators who have a Bachelor's degree must have three (3) years of experience providing case management/service coordination, information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.
Persons self-employed or employed as Service Coordinators who have an Associate's degree must have five (5) years' experience providing case management/service coordination, information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors.

And Must have:

Experience working with individuals with disabilities, seniors, or Medicaid populations strongly preferred
Knowledge of the NHTD Waiver Program, Medicaid, and community-based services is a plus
Strong organizational, communication, and documentation skills
Ability to work independently and manage a caseload effectively
Proficiency in Microsoft Office and electronic documentation systems
Valid driver’s license and reliable transportation preferred
Bilingual Russian Speaking Required

Pay: $27.00 - $32.00 per hour
