Full Job Description

PRIMARY FUNCTION: Leads a small team, coordinating care, and providing guidance to clients with complex psychiatric and/or co-morbid medical conditions. Develops effective interpersonal relationships with clients and works collaboratively with the interdisciplinary care team, most often onsite at an outposted hospital or clinic location, to improve health outcomes. Utilizes internal and community resources to educate clients and form a care plan with specific health outcomes. Begins the process of learning the Health Home Administrative processes.

DUTIES AND RESPONSIBILITIES:

Responsible in supervising a small Health Home Team or Peer Interns, while carrying a small caseload. Hold weekly supervision with this team, either individually or as a team. Conduct all required evaluations on the employees that report to you.
Utilizes approved Health Home assessment instruments to prepare initial and ongoing clinical and psychosocial evaluations of mental health, health and other related service needs of identified patients. Confirms the acuity level of identified patient and tailors a care-plan accordingly, with reassessments as needed.
Develops, coordinates and integrates a written, coordinated care plan in cooperation with the patient, the patient’s family, and/or the other providers serving the patient. Updates plan at specified intervals, and as needed based on changes in patient’s condition / circumstances.
Performs and maintains effective care management for a small caseload as assigned, from assessment to case closure. Tracks/ monitors client progress and produces/maintains detailed, accurate and timely case notes. Facilitates periodic case record reviews and case conferences with all providers serving the patient. Reviews new cases for completeness of documentation.
Tracks all medical, behavioral substance use and other network referrals made for clients and ensures clients follow up on referrals and attend scheduled appointments, through accompaniment when necessary.
Provides phone and physical outreach to clients who have been non-adherent to necessary treatment appointments or have missed appointments for initial visits with new providers.
Provides outreach via phone to clients to ensure appropriate follow up regarding self-care, medication refills, Care Plan adherence, scheduled office visits, test results/lab work, and all other pertinent psycho-social issues.
Ensures that relevant care team members receive important client alerts, including ER visits, hospitalization admission/discharge information, and other urgent care notifications.
Tracks/monitors client progress through a combination of written work, agency databases, health home data system and case conferences with the Care Team. Documents and maintains case records in an agency database and completes all data entry in a timely fashion.
Responsible for Health Home Administrative duties such as checking and reviewing Health Home team HML, Case Notes and providing monthly data to their AD for billing reporting.
Represents Alliance with the highest level of professionalism and respect within hospital and clinic setting, providing care management services to patients on site and in the field.
Monitors client entitlements, insurance, and other benefits to ensure they remain active and in place.
Participates in initial and ongoing training as necessary to maintain a level of knowledge related to serious physical ailments as defined by Health Home regulations.
Attend regular supervision, departmental, agency, or any other meetings as directed.
Attend a minimum of two, in-person agency fundraising events, advocacy events, or events for participants (e.g., Peer Graduation, Voices Poetry Reading, World AIDS Day, AIDS Walk etc.) every 12-month period.
Contribute to agency fundraising efforts by committing $150 in “give or get” (donated or fundraised through network) every 12-month period.
Commitment to continued learning.
Other duties as assigned and required to achieve the goals, objectives, the long-term vision, and sustainability of Alliance.

QUALIFICATIONS: The Senior Health Home Care Manager will have a bachelor’s degree and have spent at least three years providing direct care management services to clients. The SCM should ideally have an advanced knowledge of HIV/AIDS, Substance Abuse, Mental Health, and Entitlement issues.Multilingual (particularly Spanish speaking) candidates are preferred. Commitment to fostering a culture of accountability, attention to quality, and inclusivity. Authorized to work in the United States. Current COVID-19 vaccination required. Flu shot preferred.

Pay: $55,000.00 per year
