Registered Nurse (RN) – Home Health (Medicare Certified)

Location: Minneapolis–St. Paul Metropolitan Area, Minnesota
Employment Type: Part-Time (Opportunity for Full-Time)
Pay: $41.83–$45.38 per hour (Based on experience)

Job Overview

Elizjoseph Healthcare LLC is seeking an experienced, compassionate, and dependable Registered Nurse (RN) to join our growing Medicare-certified Home Health Agency. The RN will provide skilled nursing services to clients in their homes throughout the Minneapolis–St. Paul metropolitan area.

The ideal candidate will have prior Medicare Home Health experience and be proficient in OASIS assessments, Medicare Conditions of Participation (CoPs), care planning, and home health documentation. This position is ideal for a nurse who is committed to delivering high-quality, patient-centered care while helping clients remain safely and independently in their homes.

Responsibilities

Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
Perform comprehensive skilled nursing assessments and routine home visits.
Develop, implement, and revise individualized plans of care in collaboration with physicians and the interdisciplinary team.
Administer medications, treatments, injections, wound care, catheter care, and other skilled nursing services as ordered.
Monitor patients for changes in condition and communicate significant findings promptly to the physician and care team.
Educate patients and caregivers on disease management, medications, safety, and self-care.
Ensure compliance with Medicare regulations, state and federal requirements, and agency policies.
Complete accurate and timely documentation in the agency's electronic medical record.
Participate in quality assurance and performance improvement activities.
Coordinate care with physicians, therapists, home health aides, social workers, and other healthcare professionals.

QualificationsRequired

Current Registered Nurse (RN) license in the State of Minnesota.
Minimum of one (1) year of Home Health nursing experience.
Experience completing Medicare OASIS assessments is required.
Strong knowledge of Medicare Home Health regulations and documentation requirements.
Current BLS/CPR Certification.
Valid driver's license, reliable transportation, and current automobile insurance.
Ability to travel throughout the Minneapolis–St. Paul metropolitan area.

Preferred

Experience with electronic home health documentation software.
Wound care experience.
IV therapy and infusion experience.
Psychiatric or behavioral health nursing experience.
Case management experience.

Schedule

Part-Time
Minimum of three (3) shifts per week
Flexible scheduling
Weekend availability may be required
