Travel RN Case Manager | Monday–Friday | Atlanta, GA | 12-Week Contract

Job Description:

We are seeking an experienced Travel Registered Nurse (RN) Case Manager for a 12-week assignment in Atlanta, GA. This is a great opportunity for a skilled case management professional who excels in discharge planning, care coordination, and ensuring quality patient outcomes in an acute care setting.

Position Details:

Position: Travel RN Case Manager
Location: Atlanta, GA 30309
Contract Length: 12 Weeks
Schedule: Monday–Friday
Shift: Days – 5x8-Hour (8:00 AM – 4:30 PM)
Weekend Requirement: None
On-Call: None

Responsibilities:

Assess, evaluate, and coordinate patient discharge plans.
Facilitate appropriate transitions of care and coordinate post-discharge services.
Collaborate with physicians, nursing staff, and interdisciplinary teams to ensure optimal patient outcomes.
Monitor patient progress daily and identify barriers to discharge.
Review length of stay and implement strategies to support timely and safe discharges.
Ensure compliance with Medicare regulations, including patient choice requirements, IMM (Important Message from Medicare) notices, and MOON (Medicare Outpatient Observation Notice) documentation.
Maintain accurate documentation and communicate effectively with patients, families, and healthcare providers.

Minimum Qualifications:

Active Registered Nurse (RN) license.
Minimum of 3 years of recent RN Case Management experience.
Strong experience with discharge planning, utilization review, and care coordination.
Knowledge of Medicare guidelines, including IMM and MOON requirements.
Excellent communication, organizational, and critical thinking skills.
