Full Job Description

Silver Oak Nursing & Rehabilitation Center – Marion, IA

Silver Oak is seeking an experienced, organized, and relationship-focused Admissions Coordinator to join our leadership team.

This role serves as the primary liaison between hospitals, referral sources, residents, families, and Silver Oak throughout the admissions process. From the first referral to signing admission agreements and ensuring a smooth transition into the facility, you'll play a vital role in helping residents find the care they need while supporting the continued growth of our community.

Schedule

Full-time
Primarily weekday business hours
Occasional evening community networking events as needed
Rare weekend availability may be required to coordinate resident admissions or hospital readmissions

Responsibilities

Serve as the primary point of contact for hospitals, discharge planners, physicians, case managers, families, and other referral sources.
Coordinate all aspects of the admissions process from initial referral through resident admission.
Review incoming referrals for clinical and financial appropriateness in collaboration with nursing leadership and the Business Office.
Conduct tours for prospective residents and families.
Complete admission agreements and all required admission documentation with residents and responsible parties.
Maintain accurate bed availability and communicate admissions, transfers, and discharges with all departments.
Coordinate room selection with Nursing, Housekeeping, and other departments to ensure smooth admissions.
Build and maintain strong relationships with hospitals, physician offices, home health agencies, hospice providers, assisted living communities, and other referral partners.
Represent Silver Oak at community networking events, including the Elderly Consortium, Marion Chamber of Commerce, and other healthcare and community organizations.
Perform routine marketing visits to hospitals, physician offices, and referral sources throughout the area.
Assist with occupancy goals while ensuring every admission is clinically appropriate and in the resident's best interest.
Maintain knowledge of Medicare, Medicaid, managed care, and other payer sources as they relate to admissions.

Qualifications

Previous healthcare experience is required.
Licensed Nurse (RN or LPN) strongly preferred.
Previous admissions, marketing, case management, discharge planning, or business development experience in a skilled nursing facility, hospital, home health, hospice, or other healthcare setting is preferred.
Experience using Epic, CareLink, or other electronic referral systems is preferred, but not required.
Exceptional communication and relationship-building skills.
Strong attention to detail and excellent organizational skills.
Ability to prioritize multiple responsibilities in a fast-paced environment.
Professional, compassionate, and customer-focused demeanor.
Valid driver's license and willingness to travel locally for hospital visits, community events, and routine marketing activities.
