Full Job Description

Job Summary

Reformed Church Home is a 5-Star Assisted Living and Skilled Nursing Home located in Old Bridge, New Jersey. We are actively seeking a full time RN, 11:00PM - 7:00AM shift. Candidate will also act as Nursing Supervisor on a rotating basis.

Responsibilities and Duties

Under the direction of the Unit Manager you will be responsible to provide residents of the facility with optimal care. General duties will include:

Organize and supervise Certified Nursing Assistants rendering resident centered care with the concept of 24-hour continuity as a goal.
Administer medication and treatment to residents and provides input to the Unit Managers, Supervisor or physician with regards to therapeutic value of each.
Document appropriate happenings in all designated areas (nursing notes, incident reports, care plans, 24-hour report) so all shifts and disciplines will have information pertinent to those residents.
Communicate with Unit Managers, Supervisors, and all disciplines, any changes regarding resident status which may initiate a change in that resident’s plan of care, or institute emergency evaluation of the resident.
Serve, assist and assure completion of admissions, discharges, etc. in conjunction with the Unit Manager and/or supervisors.
Performs skilled nursing procedures as required.

Qualifications and Skills

Candidate must be a Registered Nurse licensed by the State of New Jersey or possess multi-state license. Supervisory experience required.
