Full Job Description

Presentation Skilled Nursing and Rehab is a premier long-term care and skilled nursing facility committed to delivering exceptional care. We are seeking a dedicated and experienced MDS Coordinator to join our team, ensuring high-quality care planning, regulatory compliance, and resident satisfaction.

Pay: 50-55/hr. DOE

Job Summary
The MDS Coordinator/Care Planning Nurse is responsible for managing the Resident Assessment Instrument (RAI) process, including Medicare/PPS and OBRA assessments, to ensure accurate coding and care plan development. The successful candidate will chair interdisciplinary team (IDT) meetings, maintain compliance with federal and state regulations, and ensure revenue optimization through accurate documentation.

Essential Responsibilities

IDT Leadership: Chair Medicare IDT meetings and resident conferences (OBRA & PPS) in accordance with mandated time frames.
Care Planning: Develop and maintain written interdisciplinary care plans for all residents, serving as the liaison between the facility, family, and residents.
MDS Management: Coordinate, complete, and transmit MDS assessments, maintaining data accuracy and integrity.
Quality Indicators: Monitor Quality Measures and Quality Indicators, identifying areas for process improvement related to resident assessment/care planning.
Documentation Liaison: Communicate with charge nurses, nursing managers, and the DNS regarding documentation and clinical issues.
Compliance: Maintain compliance with all regulatory requirements and the RAI completion process.
Admissions & Screening: Screen potential new admissions for appropriate level of care.
Customer Service: Provide excellent customer service to families, residents, and staff.

Qualifications

Licensure: Valid MA RN license or LPN MA license required.
Experience: One (1) year of experience with Resident Care Planning, MDS, and Medicare/PPS preferred.
Knowledge: Knowledge of federal and state regulations, OBRA, and PPS.
Skills: Strong organizational, communication, and interdisciplinary team leadership skills.
Experience: MMQ experience is a plus.
