Full Job Description

About the Position

Evergreen Adult Day Health Center is seeking a compassionate, dependable, and organized Licensed Practical Nurse (LPN) to join our clinical team.

The LPN will provide nursing care and health monitoring for adults attending our Adult Day Health Center. Under the supervision of a Registered Nurse or Clinical Nurse Manager, the LPN will administer medications, monitor participants’ health conditions, provide treatments, complete clinical documentation, and communicate changes in condition to the appropriate healthcare professionals.

This is an excellent opportunity for a nurse who enjoys developing meaningful relationships with older adults and adults with medical, cognitive, or functional needs. Our program operates primarily during daytime hours, with no overnight shifts.

Responsibilities

Administer medications accurately and safely according to healthcare provider orders.
Monitor participants’ vital signs, blood glucose, weight, oxygen saturation, pain levels, and general health status.
Observe participants for changes in physical, cognitive, behavioral, or emotional condition.
Promptly report significant changes or clinical concerns to the supervising Registered Nurse.
Provide treatments and nursing interventions within the LPN’s scope of practice.
Assist with participant admissions, reassessments, care plans, and discharge processes as assigned.
Maintain accurate medication administration records, progress notes, treatment records, and other required clinical documentation.
Communicate with participants, caregivers, healthcare providers, pharmacies, and members of the interdisciplinary team.
Respond appropriately to participant illnesses, injuries, falls, and medical emergencies.
Follow infection-control, medication-management, participant-safety, HIPAA, and emergency-preparedness procedures.
Participate in interdisciplinary meetings, staff training, quality-improvement activities, and clinical audits.
Provide health education and reinforce instructions regarding medications, nutrition, hydration, diabetes management, fall prevention, and chronic-disease management.
Assist participants with personal care, mobility, transfers, or activities of daily living when necessary.
Maintain a respectful, supportive, culturally sensitive, and participant-centered environment.
Perform other nursing duties assigned by the Registered Nurse or Clinical Nurse Manager that are within the LPN’s scope of practice.

Qualifications

Current and unrestricted Massachusetts Licensed Practical Nurse license.
Graduate of an accredited practical nursing program.
Current CPR/BLS certification.
At least one year of nursing experience preferred.
Experience working in adult day health, long-term care, assisted living, home care, rehabilitation, community health, or geriatrics is preferred.
Experience caring for older adults and individuals with chronic medical conditions, disabilities, dementia, or behavioral health needs is preferred.
Knowledge of medication administration, infection prevention, emergency procedures, and clinical documentation.
Ability to recognize and promptly report changes in a participant’s condition.
Strong communication, organizational, and interpersonal skills.
Ability to work effectively as part of an interdisciplinary team.
Basic computer skills and ability to use an electronic health record.
Ability to maintain participant confidentiality and comply with HIPAA requirements.
Dependable attendance, punctuality, and professional conduct.
Ability to successfully complete required background checks, health screenings, and employment verifications.

Preferred Skills

Experience working with culturally and linguistically diverse populations.
Experience with medication administration records and electronic clinical documentation.
Strong assessment, observation, and time-management skills.

Physical Requirements

Ability to stand and walk for extended periods.
Ability to bend, reach, kneel, and perform routine nursing tasks.
Ability to assist participants with transfers, mobility, positioning, and personal care.
Ability to lift or move up to [25–50] pounds with assistance or appropriate equipment.
Ability to respond promptly during medical or participant-safety emergencies.
