Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with expertise in various clinical settings, including long-term care, assisted living, and acute care environments. As an LPN, you will play a vital role in delivering high-quality nursing services, supporting patient recovery, and ensuring compliance with healthcare regulations. Your proactive approach and excellent communication skills will contribute to creating a safe, respectful, and supportive environment for our patients and their families.

Duties

Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems.
Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA and clinical confidentiality policies.
Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
Provide wound care, catheterization, IV infusion therapy, and ventilator management as required for various patient populations.
Support patient mobility and comfort through activities such as tube feeding, bed positioning, and the use of assistive devices like Hoyer lifts.
Collaborate with interdisciplinary teams including physicians, case managers, and caregivers to develop personalized care plans aligned with long-term care regulations.
Educate patients and families on health management strategies including pain management, dementia care (including Alzheimer’s and memory care), and discharge planning.
Maintain accurate medical documentation of patient care activities in EMR/EHR systems while ensuring compliance with all safety standards.

Requirements

Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state or regional licensing boards.
Proven experience working in diverse healthcare settings such as skilled nursing facilities, hospitals (including ICU or trauma centers), hospice care, or outpatient clinics.
Demonstrated proficiency with EMR/EHR systems such as Cerner or Athenahealth; experience with ICD-9/ICD-10 coding is preferred.
Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment.
Experience working with geriatric patients or individuals with developmental disabilities is highly desirable.
Ability to perform clinical procedures including venipuncture, suturing assistance, sterile technique, and first aid.
Excellent communication skills for effective caregiver communication and patient education in a respectful manner.
Knowledge of infection control practices and long-term care regulations to ensure safety standards are met consistently.
Certification in Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR); additional certifications such as IV insertion or advanced life support are advantageous.
