Schedule: Monday–Friday | 8:00 AM – 2:00 PM (30 Hours per Week)
Pay: $18.00–$21.00 per hour (Based on Experience)

Full Circle Home Care is a growing Maryland-based provider of Home Care and Developmental Disabilities Administration (DDA) services dedicated to Creating Healthcare Solutions for the Community.

Position Summary
Full Circle Home Care is seeking a dependable and organized Care Coordinator to support the daily operations of our Home Care and DDA programs.
This role extends beyond traditional care coordination and includes client services, recruitment, interviewing, staffing coordination, compliance, quality assurance, and office operations.
Working closely with agency leadership and our Virtual Assistant, the Care Coordinator helps ensure exceptional service delivery while maintaining regulatory compliance and supporting the continued growth of the organization.

ResponsibilitiesClient & Care Coordination
- Coordinate services for new and existing clients.
- Assist with caregiver and DSP scheduling.
- Confirm staffing coverage and communicate schedule changes.
- Conduct follow-up calls with clients, families, and caregivers.
- Address service concerns and escalate issues when appropriate.
- Support continuity of care for all assigned clients.
Recruitment & Interviewing
- Recruit, screen, and conduct initial interviews for caregivers and Direct Support Professionals (DSPs).
- Coordinate interviews with agency leadership.
- Assist with onboarding and orientation scheduling.
- Monitor applicant progress and follow up on missing employment requirements.
- Build positive relationships with prospective employees throughout the hiring process.
Compliance, Quality Assurance & File Management
- Maintain organized electronic and paper client and employee files.
- Review records for completeness, accuracy, and compliance.
- Monitor employee credentials, licenses, CPR certifications, physicals, TB testing, and other required documentation.
- Assist with quality assurance audits.
- Prepare records for state surveys, licensing reviews, and compliance audits.
- Identify missing documentation and coordinate corrective action.
Office Operations
- Answer incoming phone calls and respond to agency emails.
- Welcome visitors and provide outstanding customer service.
- Maintain organized office records and supplies.
- Assist leadership with daily operational needs and special projects.
- Support communication between clients, families, staff, and leadership.
Community Outreach
- Assist with recruitment events and community outreach initiatives.
- Support hiring efforts and agency marketing.
- Help coordinate newsletters, events, and community engagement activities.
Operational Support
Full Circle Home Care is a team-oriented organization where continuity of care is our highest priority.
While this position is primarily office-based, team members are expected to support the overall success of the organization. Responsibilities may include assisting with scheduling, administrative projects, recruitment efforts, and other operational needs.
In rare situations involving urgent staffing needs, the Care Coordinator may be asked to provide direct caregiver or Direct Support Professional (DSP) support, if qualified and available, to help ensure uninterrupted services for our clients.
Qualifications
- High school diploma or GED required.
- Associate's or Bachelor's degree preferred.
- Previous experience in home care, healthcare, or human services preferred.
- Caregiver, CNA, GNA, CMT, DSP, staffing, or scheduling experience strongly preferred.
- Previous interviewing or recruiting experience is a plus.
- Strong organizational and time management skills.
- Excellent verbal and written communication skills.
- Ability to multitask in a fast-paced environment.
- Professional customer service skills.
- Experience with Microsoft Office and Google Workspace.
- Experience with CareSmartz360 or another EVV system preferred.
- Valid driver's license and reliable transportation required.
