The Brain Injury Association of Iowa (BIAIA) is seeking a compassionate, organized, and self-motivated part-time Neuro Resource Facilitator to join our team!
This is a remote, work-from-home position. Applicants must reside in Iowa. BIAIA provides the necessary computer, communication, and office equipment. While the position is primarily remote, occasional travel throughout Iowa is required for meetings, outreach, training, and other program activities.
About the Brain Injury Association of Iowa
Since 1981, the Brain Injury Association of Iowa has worked to improve the lives of Iowans affected by brain injury through support, advocacy, education, prevention, and research.
Since 2007, BIAIA has provided Brain Injury Resource Facilitation (BIRF), a person-centered, evidence-based service that helps individuals with brain injury and their families navigate complex systems of care, access community resources, and achieve their personal goals.
As a Neuro Resource Facilitator, you will provide information, education, advocacy, and ongoing support to individuals with brain injury, their families, and professionals throughout Iowa.
Neuro Resource Facilitation Job Description
Reports to: Project Director
Compensation: $20-$24/hour
Anticipated Hours: 20 hours/week
Primary Responsibilities:
Direct Services:
- Provide person-centered resource facilitation services to individuals with brain injury and their families.
- Conduct consultations to identify strengths, concerns, goals, and service needs.
- Assist individuals in accessing community resources, benefits, healthcare, and disability services.
- Support individuals in developing strategies to achieve personal goals and increase independence.
- Promote self-management of brain injury as a chronic condition.
- Advocate for equitable access to services and community inclusion.
- Coordinate and monitor services to ensure they are appropriate, timely, and effective.
- Conduct ongoing assessments and update service plans as needed.
- Maintain regular follow-up with individuals and families throughout the resource facilitation process.
- Collaborate with discharge planners, healthcare providers, and community partners.
- Maintain accurate, timely, and confidential client documentation in accordance with organizational policies.
- Balance service intensity and duration based on individual needs.
Indirect Services:
- Review referrals and determine program eligibility.
- Collect program outcome data to support quality improvement and reporting.
- Develop and maintain relationships with healthcare providers, rehabilitation programs, disability service organizations, and other community partners.
- Promote Brain Injury Resource Facilitation through outreach, presentations, and community education.
- Participate in trainings, workshops, conferences, advocacy activities, and continuing education.
- Collaborate with the Neuro Resource Facilitation team to strengthen program quality and service delivery.
- Contribute to continuous quality improvement initiatives and organizational projects.
- Perform other duties as assigned.
Qualifications:
- Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a closely related field, or a minimum of two years of direct human services experience working with individuals with disabilities; experience supporting individuals with brain injury is strongly preferred.
- Knowledge of Iowa's disability service system and community resources.
- Excellent written, verbal, and interpersonal communication skills.
- Strong organizational, time management, and computer skills.
- Ability to work independently in a remote environment while collaborating effectively as part of a team.
- Valid driver's license and the ability to travel independently throughout Iowa.
- Certified Brain Injury Specialist (CBIS) preferred.
