POSITION SUMMARY:
The Primary Source Verifier (PSV) is responsible for ensuring all practitioner licensure and credentialing requirements are verified through appropriate primary sources and completed prior to expiration. This role is critical to maintaining compliance with regulatory and accreditation standards, including Joint Commission requirements. The PSV serves as the designated owner of all Primary Source Verification activities, ensuring accurate tracking, timely completion, and appropriate escalation of any compliance risks.
POSITION EDUCATION/ QUALIFICATIONS:
- Minimum of High School Diploma or GED
- Experience in credentialing, medical staff services, or healthcare administration preferred
- Knowledge of credentialing systems (e.g., Symplr) preferred
- Understanding of primary source verification (PSV) processes preferred
- Proficiency in Microsoft Office and database systems

Responsibilities:
POSITION RESPONSIBILITIES:
· Perform primary source verification (PSV) for Physicians (MD/DO/DDS/DPM) and Advanced Practice Providers (APPs), including licensure, DEA/DPS, board certification, education, training, and work history.
· Ensure timely completion of PSV activities by monitoring license expiration dates and verification status; complete all verifications within required timeframes.
· Document all verification activities accurately in credentialing systems and provider files, including maintaining complete and audit-ready records of both system-generated and manual verifications.
· Utilize Symplr credentialing software for tracking, documentation, and workflow management; recognize system limitations and ensure appropriate manual processes are followed when verifications cannot be completed within the system.
· Perform manual primary source verification for licensing boards or entities not supported by system tools (e.g., Texas Board of Nursing), ensuring documentation is complete and compliant.
· Review verification results for discrepancies, inconsistencies, or gaps, and follow up with primary sources, providers, or departments to obtain missing or incomplete information.
· Issue expiration reminders at defined intervals and follow up to ensure timely renewal and compliance.
· Identify, escalate, and document issues that may pose a compliance risk, including notifying Medical Staff leadership as appropriate.
· Apply hard stop controls for providers with incomplete or expired required documentation (e.g., malpractice coverage, temporary privileges).
· Maintain compliance with regulatory and accreditation standards (e.g., Joint Commission, CMS) and support internal audits, compliance reviews, and accreditation surveys.
· Coordinate with internal departments including, Renaissance Medical Foundation (RMF), Human Resources, PHO, and Compliance to ensure complete and accurate credentialing information.
· Maintain confidentiality and security of sensitive provider information in accordance with organizational policies.
· Participate in Joint Commission and regulatory agency audits.
· Follow established credentialing policies and procedures and recommend process improvements to enhance efficiency, accuracy, and compliance.
· Performs other duties as assigned to support departmental operations and organizational needs.

B. Working Conditions : The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
C. Occupational Exposure: This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids). Person who performs these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way.
