Full Job Description

Job Summary

The Hospice Physician Liaison is responsible for developing and maintaining professional relationships with community physicians, primary care providers, specialists, and their office staff to promote hospice services and increase appropriate patient referrals. This position serves as the primary point of contact for physician offices, providing education on hospice eligibility, services, and referral processes while ensuring exceptional customer service and ongoing communication.

Essential Duties and Responsibilities

Develop and maintain productive relationships with physicians, nurse practitioners, physician assistants, and office personnel within assigned territories.
Conduct routine in-person visits to physician offices to educate providers about hospice services, eligibility criteria, and the benefits of timely hospice referrals.
Identify opportunities to increase hospice referrals through strategic relationship development and consistent provider engagement.
Serve as a trusted resource regarding hospice regulations, admission criteria, payer guidelines, and available clinical services.
Provide educational presentations and in-service training to physician offices regarding hospice care and end-of-life services.
Respond promptly to physician inquiries and coordinate with hospice clinical staff to facilitate referrals and admissions.
Collaborate with the admissions, intake, and clinical teams to ensure seamless communication with referring providers.
Maintain accurate records of physician contacts, referral activities, and outreach efforts using the organization's CRM or referral tracking system.
Monitor referral patterns and identify opportunities for practice growth and market development.
Represent the hospice professionally while maintaining compliance with all federal, state, and organizational policies, including HIPAA regulations.
Attend physician office meetings, networking events, and community healthcare functions as appropriate.
Meet established outreach, relationship-building, and referral growth objectives.

Qualifications

Bachelor's degree in Healthcare Administration, Business, Marketing, Communications, or a related field preferred.
Minimum of two years of healthcare sales, physician liaison, hospice, home health, or medical marketing experience preferred.
Knowledge of hospice philosophy, Medicare hospice benefit, and hospice eligibility criteria preferred.
Valid driver's license with reliable transportation and the ability to travel throughout the assigned territory.

Required Skills

Ability to educate healthcare professionals on hospice services and clinical benefits.

Working Conditions

Frequent local travel to physician offices within the assigned territory.
