Full Job Description

Job Overview
We are seeking a highly skilled and compassionate Nurse Practitioner specializing in Home Health and Hospice care. This role involves providing comprehensive patient assessments, managing complex medical conditions, and delivering personalized care in patients' homes and hospice settings. The ideal candidate will possess extensive clinical expertise across various specialties, including critical care, geriatrics, palliative medicine, and pediatric care. Experience with EMR/EHR systems is essential. The Nurse Practitioner will collaborate with interdisciplinary teams to ensure high-quality, patient-centered care while adhering to HIPAA regulations and best practices in infection control. This position offers an opportunity to make a meaningful impact on patients' quality of life through compassionate and expert medical services.

Responsibilities

Conduct thorough patient assessments, including vital signs, physical examinations, and diagnostic evaluations.
Develop and implement individualized care plans aligned with patient needs and goals.
Administer medications, injections, IV infusions, and perform procedures such as suturing, catheterization, spinal taps, and gastrostomy tube management.
Manage acute pain and chronic conditions within the scope of practice, including hospice and palliative medicine cases.
Utilize EMR/EHR systems
Coordinate discharge planning, case management, utilization review, and outpatient or inpatient follow-up care.
Provide health coaching and education tailored to diverse populations including geriatrics, pediatrics (including neonatal ventilator management), memory care patients with Alzheimer’s or dementia, individuals with developmental disabilities or disabilities.
Collaborate with specialists in areas such as infectious disease care, occupational health, behavioral health, cardiology (including cardiac catheterization), ophthalmology, gastroenterology (including gastric lavage), and respiratory therapy (ventilator management).
Participate in telehealth consultations when appropriate to extend access to specialized care.
Maintain compliance with HIPAA standards and infection control protocols while working in sterile environments or aseptic techniques during procedures.

Skills

Proven experience in diverse clinical settings such as hospital medicine (Level I & II trauma centers), emergency rooms, ICU/PICU units, labor/delivery units (neonatal ventilator), outpatient clinics, or post-acute care environments.
Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth; familiarity with medical documentation standards such as ICD coding (ICD-10/ICD-9) and CPT coding.
Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), medical terminology, and diagnostic evaluation techniques.
Skilled in performing procedures like venipuncture/phasbotomy/injections/IV insertion/sterile processing/catheter care/airway management/oxygen therapy/tube feeding.
Ability to work independently in home health settings while coordinating with interdisciplinary teams for discharge planning and case management.
Excellent communication skills for patient education across age groups—from infants to seniors—and working effectively with families or caregivers.
Knowledge of Workers' Compensation law applicable to occupational health cases is a plus.
