Full Job Description

Job Title: Discharge Planning Coordinator

Pay: $25-$28/hr

Location: Remote, PST or MST CANDIDATES ONLY

Type: Contract – 26 weeks (possible temp-to-perm)

Schedule: M-F 7:30a-3:30p PST, rotating every couple of weekends required

Minimum Qualifications

Minimum 1 year of recent hospital Care Coordination, Case Management Support, Transition of Care, Discharge Planning, Utilization Management, or Patient Placement experience
Experience coordinating post-acute referrals, authorizations, and discharge arrangements
Experience working with SNFs, home health agencies, rehab facilities, LTACHs, or other post-acute providers
Epic experience required
Experience with UR transmissions required

Preferred Qualifications

Experience supporting Transition of Care (TOC) or discharge planning workflows
Experience coordinating referrals, authorizations, and post-acute placements
Experience with Allscripts or similar EMR systems

Position Summary

The Discharge Planning Coordinator supports care coordination and Transition of Care (TOC) discharge planning for ambulatory patients. This role partners closely with Case Managers and Social Workers to ensure timely, efficient patient transitions across the care continuum. Responsibilities include coordinating referrals, managing high-volume administrative workflows, and maintaining accurate documentation in Epic and other systems.

Key Responsibilities

Support discharge planning and care coordination activities to ensure timely patient transitions
Coordinate referrals, authorizations, and post-acute services in collaboration with Case Managers and Social Workers
Manage high-volume fax, voicemail, and data entry tasks with accuracy and efficiency
Document care coordination activities and patient updates in Epic and other EMR systems
Assist with patient transfers to post-acute facilities and prepare required documentation
Communicate with internal teams and external providers regarding patient status and care plans
Participate in daily huddles and escalate barriers to discharge as needed
