Full Job Description

Position Summary (Registered Nurse/ RN)

We are seeking an experienced Registered Nurse (RN) to serve as the nursing leader for a 4-home Intermediate Care Facility for individuals with Developmental Disabilities (ICF/DD) program, supporting a total of 24 clients. This is an in-person, on-site role and is not eligible for remote work. This role supervises and educates LVN staff, and requires strong coaching skills, sharp clinical assessment and triage ability, in-depth knowledge of intellectual and developmental disabilities (IDD), and a willingness to learn ICF/DD-specific regulations.

Program Overview (Registered Nurse/ RN)

The program consists of four licensed ICF/DD group homes located within 2 miles of one another in Merced, collectively serving 24 individuals with intellectual and developmental disabilities. The RN will work closely with LVNs, House Managers, QIDPs, Direct Support Professionals, physicians, and the interdisciplinary team to ensure high-quality, person-centered nursing care across all four homes.

Key Responsibilities (Registered Nurse/ RN)

Provide clinical nursing leadership and oversight across all 4 homes (24 clients total).
Supervise, coach, and educate LVNs and direct support staff on nursing procedures, clinical protocols, and best practices in IDD care.
Complete and maintain individual health assessments, nursing care plans, and physician orders.
Perform and oversee clinical assessment and triage of client health needs, determining appropriate level of response and follow-up.
Oversee medication administration, medication reviews, and delegation of nursing tasks to LVNs and trained direct support staff.
Ensure compliance with Title 22, Community Care Licensing, and all applicable ICF/DD state and federal regulations.
Visit each home on a weekly, in-person basis to provide hands-on assessment guidance and training to LVNs and direct support staff.
Supervise and guide the program through regulatory surveys (e.g., Title 22, Community Care Licensing, CMS), ensuring survey readiness and appropriate follow-up on findings.
Coordinate care with physicians, specialists, regional center service coordinators, and families.
Respond to urgent health needs.
Travel daily, in person, to conduct site visits, staff check-ins, and client care.
Maintain accurate, timely documentation in health records and electronic charting systems.
Support incident reporting, quality assurance, and health and safety compliance across the program.

Qualifications (Registered Nurse/ RN)

Active, unrestricted Registered Nurse (RN) license in the State of California.
Prior experience working with individuals with intellectual and/or developmental disabilities (IDD) required.
Demonstrated leadership or supervisory experience, including supervising and educating LVNs, in a healthcare, residential, or ICF/DD setting.
Strong coaching and staff education skills, with the ability to train and mentor LVNs and direct support staff.
Skilled in clinical assessment and triage.
Experience supervising or guiding a facility through a regulatory survey (Title 22, Community Care Licensing, CMS, or similar).
Willingness to learn and apply ICF/DD regulations, Title 22, and Community Care Licensing standards.
Comfortable and skilled with computers, including electronic health records (EHR) systems and standard office software.
Able to work in person and travel daily/weekly between homes; this role is not eligible for remote work.
Current CPR/First Aid certification.
Strong communication, organizational, and interdisciplinary collaboration skills.

Location Requirement (Registered Nurse/ RN)

Must reside within 15 miles of Merced, CA.
Must be able to commute daily within the Merced service area.
Reliable transportation and a valid driver's license are required.

Pay: From $102,000.00 per year
