Now Hiring! Bilingual HCBS Program Coordinator

Position Summary
Are you a compassionate, organized professional looking to make a meaningful impact in your community? Join a team dedicated to supporting individuals with developmental and physical disabilities to live independently and thrive. As an HCBS Program Care Coordinator, you’ll play a vital role in connecting members to quality care, supporting Direct Care Workers (DCWs), and building strong relationships with families and partners. This is an excellent opportunity for both growing professionals and experienced case managers seeking purpose-driven work in a collaborative, supportive environment.

In this role, you will support the day-to-day operations of Home and Community-Based Services (HCBS), ensuring high-quality care delivery, workforce coordination, and compliance with regulatory standards.

Key Responsibilities

Supervise and support Direct Care Workers (DCWs), including scheduling, onboarding, and performance oversight
Ensure adequate staffing coverage and provide direct support as needed
Monitor timesheets, payroll accuracy, and service documentation
Maintain compliance with DDD regulations, EVV requirements, and agency standards
Coordinate member services, including intake, service planning, and ongoing support
Serve as a liaison between members, families, staff, and external partners
Participate in PCSP/ISP meetings and ensure accurate documentation and reporting
Conduct quality assurance monitoring, home visits, and staff supervision
Respond to incidents, health and safety concerns, and report per policy
Support billing, authorizations, and system tracking
Collaborate with leadership and HR on employee relations and workforce needs

Qualifications

High School Diploma or GED required; Some college or completed associate’s degree in a human services or related field preferred
Working with individuals with disabilities or related population is desired
At least 1 year of experience in case management and care coordination is required
1–2 years of supervisory experience required
Knowledge of Arizona DDD regulations a plus
Strong organizational, communication, and problem-solving skills
Ability to work independently and manage a flexible schedule

Requirements

Bilingual in Spanish is required, must be able to read and write fluently
Valid driver’s license, reliable vehicle, and auto insurance
Ability to meet AZ fingerprint clearance and direct care requirements
Proficiency in Microsoft Office applications, calendar management, cloud based and virtual platforms, and workforce management systems

Pay: $43,000.00 - $47,000.00 per year
