Overview

Join our dynamic healthcare team as a Part-Time or PRN Physical Therapist, where your expertise will directly impact patient recovery and well-being. In this role, you will provide specialized therapeutic services to primarily work with a patient population including geriatric, neurological and orthopedic. Your dedication will help patients regain mobility, reduce pain, and improve their quality of life through personalized treatment plans. This position has the option for a set part-time schedule or flexible PRN.

Responsibilities

Develop and implement individualized treatment plans based on thorough assessments of patient needs and clinical documentation standards.
Conduct patient care sessions focusing on rehabilitation, mobility improvement, pain management, and functional independence.
Collaborate effectively with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate care strategies.
Perform discharge planning to ensure seamless transition of patients from hospital or clinic to home or community settings.
Maintain accurate electronic health records (EHR) and physiotherapy documentation in compliance with medical terminology and clinical standards.

Skills

Strong knowledge of anatomy, physiology, and medical terminology relevant to physical therapy practice.
SNF experience preferred but not required
Skilled in developing effective treatment plans tailored to individual patient goals
Proficiency in electronic health records (EHR) systems and EMR (Electronic Medical Records) management for accurate documentation.
Excellent client communication skills to educate patients about their care plans and foster engagement in their recovery process.
Ability to perform discharge planning and work collaboratively with healthcare teams for comprehensive patient care.

Pay Rate will depend on experience and whether electing part-time or PRN position.

Therapy Network, Inc has provided rehabilitation services in the greater Phoenix area for nearly three decades. This position services Archstone Care Center on Dobson and Pecos. This is one of the last standalone skilled nursing facilities in Arizona, which comes with less pressure that typically comes along with working for a corporation. That being said, we still expect excellent standards of care and following job duties.

We utilize Net Health for therapy documentation along with PCC for accessing medical records.

Pay: $50.00 - $60.00 per hour
