Full Job Description

KIN CONNECT Case Manager Responsibilities

Oversight of Foster Homes for compliance

In home support for Foster Parents

Maintain Case notes for youth and foster families

Complete monthly reports and send to youth’s team.

Maintain professional relationships with Counties, GALs and treatment team members.

Aid in placement paperwork to ensure timeliness

In home visits 2 times a month.

Attend training to stay informed of best practices.

Qualifications and Education Requirements

The Case Manager must have:

Shall hold a Bachelor degree in the social or behavioral sciences from a regionally accredited college or university. If an individual has a Bachelor's degree in a non-related field, he/she shall have at least two (2) years experience supervised by an individual with a Master of Social Work or other Master's degree in the social or behavioral sciences.

Valid Colorado Driver’s License – DMV Records will be checked.

Background Clearances are required: Criminal (CBI and FBI), Child Abuse (BIU) – Negative reports may result in termination of employment.

3 written statements or telephone references as detailed in Rule 7.710.22 - 4, 5.

Submit proof of qualifications and certified transcripts from a regionally accredited college or university to the State Department within thirty (30) calendar days of accepting the position.

SAFE Certification Desired

Complete Prudent Parent Training

Additional Notes

Additional tasks as assigned. Reports to Placement Supervisor/Executive Director
