Full Job Description

CC Young, a vibrant senior living community and a non-profit leader in senior living and Health Care is seeking a compassionate, experienced Registered Nurse Case Manager to join our growing hospice team. This is an opportunity to build meaningful relationships with patients and families while making a profound difference during life's most important moments.

The Hospice RN Case Manager serves as the primary clinical coordinator for an assigned caseload of approximately 12–15 hospice patients. The RN Case Manager is responsible for comprehensive patient assessment, care planning, symptom management, family education, and coordination of interdisciplinary services.

Schedule: Monday – Friday 8:30 a.m.- 5:00 p.m. includes an on-call rotation shared among team members.

Job duties include, but are not limited to:

Manage a caseload of approximately 12–15 hospice patients.
Perform comprehensive nursing assessments and ongoing evaluations.
Develop and update individualized care plans.
Provide expert pain and symptom management.
Educate and support patients and caregivers throughout the hospice journey.
Collaborate closely with physicians, nurse practitioners, social workers, chaplains, hospice aides, and facility staff.
Coordinate medications, equipment, supplies, and community resources.
Participate in weekly Interdisciplinary Group (IDG) meetings.
Ensure accurate and timely clinical documentation.
Support patients and families through disease progression, end-of-life transitions, and bereavement preparation.
Build trusted relationships with patients and families through compassionate, patient-centered care.
Performs other duties as assigned.

Minimum qualifications include:

Valid Registered Nurse (RN) license in good standing in the State of Texas.
Minimum of 2 years of recent experience as an RN Charge Nurse, preferably in a hospice, home health, oncology, palliative care, geriatrics, skilled nursing, or critical care setting.
A valid Texas Driver’s License and reliable transportation.
Strong assessment and communication skills.
