Job Overview

We are seeking a highly skilled and dedicated Long-Term Care (LTC) Care Plan Nurse to join our healthcare team. The ideal candidate will possess extensive expertise in nursing within long-term care settings, with a focus on developing and managing comprehensive care plans that meet regulatory standards and enhance patient outcomes. This role requires a proactive approach to clinical documentation, utilization management, and interdisciplinary collaboration to ensure optimal patient care and compliance with healthcare regulations. The LTC Care Plan Nurse will play a vital role in coordinating care, reviewing medical documentation, and supporting quality improvement initiatives across the continuum of long-term care services.

Duties

Develop, implement, and regularly update individualized care plans for residents based on comprehensive assessments and clinical data.
Review medical records, documentation, and coding (including ICD-9, ICD-10, CPT) to ensure accuracy, completeness, and compliance with CMS (Centers for Medicare and Medicaid Services) regulations.
Collaborate with interdisciplinary teams—including physicians, therapists, social workers, and case managers—to coordinate patient care and discharge planning.
Conduct utilization management activities to evaluate the appropriateness of services provided and optimize resource utilization in accordance with Medicare and long-term care regulations.
Ensure adherence to clinical documentation standards, health regulation policies, HIPAA privacy rules, and state healthcare regulations.
Utilize EMR/EHR systems such as Epic or Cerner for accurate documentation review, data entry, and reporting purposes.
Provide education on clinical documentation improvement (CDI), coding practices (ICD-10/ICD-9), and regulatory compliance to staff members.
Support quality assurance initiatives by monitoring compliance with NCQA standards, MDS (Minimum Data Set) assessments, and other accreditation requirements related to long-term care facilities.

Qualifications

Registered Nurse (RN) license with current state licensure required.
Extensive experience in long-term care facilities or nursing homes; inpatient or hospital experience is highly desirable.
Strong knowledge of Medicare regulations, CMS guidelines, HIPAA compliance, and long-term care regulations.
Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks; experience with Meditech or similar platforms is a plus.
Familiarity with medical coding including ICD-10/ICD-9 codes, CPT coding, DRG (Diagnosis-Related Group), and utilization review processes.
Critical care experience—such as ICU or emergency medicine—is advantageous for managing complex cases effectively.
Excellent understanding of physiology, anatomy, medical terminology, and clinical documentation standards.
Demonstrated ability to review medical records thoroughly while ensuring compliance with healthcare policies and regulations.
Strong organizational skills with the ability to multitask efficiently in fast-paced environments while maintaining attention to detail.
Effective communication skills for collaborating across multidisciplinary teams and providing staff education on documentation practices.
