Where You Will Work:

Atrium Health Mercy is a 196-bed facility of Carolinas Medical Center (CMC). Located 1.3 miles from CMC, Mercy is one of two Carolinas Medical Center locations. Atrium Health Mercy is Magnet designated in Nursing Excellence and is also a Planetree designated patient-centered hospital. 

For more than 100 years, Atrium Health Mercy has treated approximately 80 - 110 Emergency Department and 130 - 212 in-patients daily with a special combination of human compassion and leading-edge expertise. 

Atrium Health Mercy is a full-service community hospital, specializing in the care of seniors and in complex foot and ankle surgery, hip and knee surgery, bariatric surgery, and women’s pelvic health. 

Schedule Details/Additional Information:

Full time
7 am- 3 pm, Monday thru Friday. May vary.

What You’ll Do:

Deliver diabetes self-management education and support (DSMES) in accordance with ADA and ADCES standards.
Assess patient needs, goals, and barriers to create individualized diabetes education plans.
Provide individual and group instruction on medications, nutrition, exercise, glucose monitoring, and diabetes management.
Deliver ongoing follow-up, support, and, when applicable, medical nutrition therapy services for patients with diabetes and related conditions.
Collaborate with providers and care teams to develop care plans and adjust diabetes medications per protocol, competency, and policy requirements.
Maintain expertise in diabetes technologies, current clinical guidelines, ADA standards, and regulatory requirements, while accurately documenting and billing services.
Serve as a resource for patients, providers, and the community through education programs, support groups, outreach events, and development of patient education materials.

We’re Looking For:

Licensure, Registration, and/or Certification Required: Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian, in the State of applicable state. Certified Diabetes Care and Education Specialist (CDCES) preferred. If not certified, you must obtain certification within two years preferred but not to exceed 3 years.
Education Required: Degree in Nursing, Dietetics, or other related disciplines that meet the certification eligibility requirements for CDCES. If nursing degree, BSN preferred. Master's degree in Dietetics required if Dietician license obtained after 01/01/2024.
Experience Required: 2 years of patient-oriented educational experience preferred with a broad range of knowledge on diabetes for age-appropriate population and its interrelation to co-existing health conditions.
