Job Summary
We are seeking a dedicated and highly skilled Family Nurse Practitioner (FNP) to provide comprehensive primary care services across diverse patient populations. The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and the ability to deliver compassionate, evidence-based care. As an integral member of our healthcare team, the Family Nurse Practitioner will manage patient health through preventive care, acute illness management, chronic disease treatment, and health education, ensuring optimal patient outcomes in a fast-paced environment.
Duties
- Conduct thorough physical examinations and patient assessments to diagnose acute and chronic conditions across all age groups, including geriatrics, and adult populations.
- Develop and implement individualized care plans based on clinical evaluation, utilizing evidence-based practices and current medical guidelines.
- Administer medications, including injections and IV infusions, following proper protocols and safety standards.
- Perform procedures such as suturing, catheterization, and dermal filler injections with precision and adherence to aseptic techniques.
- Order, interpret, and act on diagnostic tests, laboratory results, and imaging studies
- Utilize electronic health record (EHR) systems eClinicalWorks, for accurate documentation of patient encounters and medical records.
- Provide health coaching and education to promote lifestyle modifications, disease prevention, and management strategies tailored to patient needs.
- Develop and implement individualized, evidence-based care plans in collaboration with patients and families
- Prescribe and manage medications, including controlled substances where appropriate and within scope
- Collaborate with multidisciplinary teams in various settings including hospitals (Level I Trauma Center experience preferred), outpatient clinics, urgent care centers, nursing homes, assisted living facilities, hospice & palliative care units, and telehealth platforms.
- Engage in clinical research initiatives or quality improvement projects to enhance patient care standards.
- Demonstrate proficiency in CPT coding for billing purposes while ensuring compliance with HIPAA regulations regarding patient privacy and confidentiality.
- Participate in triage activities to prioritize urgent cases effectively while managing acute pain or infectious disease concerns.
Experience
- Proven experience as a Family Nurse Practitioner or similar role within diverse healthcare environments such as hospital medicine, emergency medicine, or outpatient clinics.
- Demonstrated expertise in managing complex cases involving critical care scenarios
- Strong background in working with special populations including, geriatrics (memory care & dementia), individuals with developmental disabilities or chronic illnesses such as dialysis patients or those requiring gastrostomy tube feeding.
- Experience utilizing EMR/EHR systems like eClinicalWorks for comprehensive medical documentation and coding (ICD-9/10).
- Ability to perform advanced assessments such as vital signs monitoring, physiology knowledge application, diagnostic evaluation techniques like gastric lavage or spinal taps.
- Required
▪ Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution
▪ Current, active Florida Nurse Practitioner license in good standing
▪ Board certification as a Family Nurse Practitioner (FNP-C or FNP-BC)
▪ Current DEA registration (or eligibility to obtain)
▪ Current BLS certification; ACLS preferred
▪ Minimum of 2 years of clinical NP experience in a primary care or family medicine setting
▪ Completion of 3,000 supervised hours required for autonomous practice under Florida law (or in progress)
Preferred
▪ Bilingual: English and Spanish (strongly preferred given South Florida patient demographics)
▪ Experience with Medicare Annual Wellness Visits and Chronic Care Management (CCM)
▪ Familiarity with value-based care models and quality metrics (HEDIS, STAR ratings)
▪ Experience building and managing a patient panel in an independent or small-group practice setting
▪ DNP degree or active pursuit of post-graduate certification
Pay: $107,706.72 - $129,711.31 per year
