This position requires a highly responsible individual who will provide integrated juvenile justice supervision and child welfare case management for youth who are dually involved in delinquency and dependency in Miami-Dade County.
This position requires a dual certification as a Juvenile Probation Officer (JPO) and Child Welfare Case Manager (CWCM) and perform all functions required under applicable Florida Statutes, Department of Juvenile Justice (DJJ)/ Department of Children and Families (DCF) administrative rules, policies, and contract requirements.
The Unified Case Manager (UCM) shall partner with an Intensive In-Home Multidisciplinary Behavioral Health Team to coordinate treatment, ensure youth compliance with court orders and supervision conditions.

SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
A. Juvenile Justice (Delinquency) Case Management & Supervision
- Performs all intake, case management, and supervision functions required of a Juvenile Probation Officer in accordance with Florida Statutes, DJJ administrative rules (including Chapter 63D-13, F.A.C.), DJJ policies, and contract requirements.
- Completes the Community Assessment Tool (CAT) Pre-Screen within 10 working days of youth placement, including an in-person motivational interview, regardless of prior assessments.
- Completes the CAT Full Assessment within 5 working days of the Pre-Screen when indicated by risk level.
- Conducts required CAT reassessments, including following new law violations, post-disposition events, and prior to 90-day supervisory reviews.
- Develops the initial Youth Empowered Success (YES) Plan within 30 calendar days of placement, ensuring required signatures (youth, parent/guardian, UCM, UCMS) and incorporates multidisciplinary input related to treatment, supervision, and care coordination.
- Updates YES Plans prior to each 90-day supervisory review and following new dispositions or violations.
- Conducts and documents required supervision contacts with youth and parents/guardians, including intensive supervision contacts, when applicable.
- Responds to technical violations using the Graduated Response Matrix (GRM) and completes required court filings (e.g., Affidavit/Petition for Violation of Probation), when indicated.
- For Supervised Release youth, coordinates with the Supervised Release Tracking (SRT) provider, ensures additional contacts for Prolific Juvenile Offenders (PJO) when applicable, files violations as required, and participates in- or ensures representation at weekly Supervised Release Review Committee (SRRC) meetings.

B. Court Reports, Screening, and Disposition Support
- Completes delinquency-related screenings and reports for new law violations, including processes involving MAYSI-2, SRSI, State Attorney Recommendation (SAR), Pre-Disposition Reports (PDR), and post-disposition reporting, when ordered.
- Follows DJJ commitment and transfer policies when residential commitment is recommended or ordered, including coordination of required staffing's and approvals through the Chief Probation Officer (CPO) process.
- Coordinates court submission processes for the 11th Judicial Circuit, ensuring accuracy and timeliness.

C. Electronic Monitoring (EM)
- Installs, activates, monitors, troubleshoots, and deactivates Electronic Monitoring (EM) equipment as ordered by the court.
- Creates and maintains EM zones and schedules; updates as needed for school changes, curfew exceptions, and court modifications.
- Responds to EM alerts and addresses noncompliance consistent with policy, including Graduated Response Matrix (GRM) responses and court filings, when required.

D. Juvenile Justice Information System (JJIS) Documentation & Data Integrity
- Maintains full compliance with JJIS documentation requirements, including:
o Case Notebook documentation
o Document Library uploads
o JJIS Face Sheet maintenance
o Youth Requirements creation and monitoring
o Adherence to required business rules and forms
- Ensures all documentation is timely, accurate, and complete, supporting supervision, court actions, referrals, and service coordination.

E. Dependency (Child Welfare) Case Management
- Provides comprehensive child welfare case management in accordance with Chapter 39, Florida Statutes; Chapter 409, Part V, Florida Statutes; Division 65C, F.A.C.; DCF policies; and CBC/Lead Agency contract requirements.
- Conducts ongoing present and impending danger assessments and implements Safety Methodology throughout the life of the case.
- Develops, implements, monitors, and adjusts behaviorally specific safety plans that addresses identified danger threats using the least intrusive means necessary to protect the child.
- Ensures safety plans remain in effect as long as danger threats exists and caregiver protective capacity is insufficient.
- Develops, implements, and monitors case plans aligned with safety threats, diminished protective capacities, conditions for return, and permanency goals.
- Conducts required face-to-face contact with children, parents, caregivers, and placements in accordance with statute and policy.
- Prepares accurate and timely judicial reviews, court reports, and permanency documentation.
- Maintains readiness for Department of Children and Families (DCF) & Community Based Care (CBC) oversight, monitoring, and quality assurance reviews.

F. Treatment Coordination & Multidisciplinary Team Collaboration
- Collaborates closely with the Intensive In-Home Multidisciplinary Behavioral Health Team, including licensed clinicians and care coordinators/life coaches.
- Participates in multidisciplinary team (MDT) meetings and Individualized Treatment Plan (ITP) reviews occurring every 60 days or less, ensuring alignment with supervision, safety planning, and dependency goals.
- Supports collection of information for monthly progress reports, discharge planning, and continuity of care.
- Ensures treatment interventions support both behavior change and risk reduction.

G. Communication, Safety, and Professional Practice
- Maintains proactive communication with Department of Juvenile Justice (DJJ), Department of Children & Families (DCF), Citrus Family Care Network (CFCN), courts, schools, caregivers, service providers, and families to align supervision, safety, permanency, and treatment goals.
Provides accurate, factual court testimony regarding supervision compliance, safety threats, protective capacities, and youth progress.
- Models ethical, trauma-informed, culturally responsive, and family-centered practice.
- Maintains confidentiality and complies with all applicable statutes, administrative rules, policies, and contractual requirements.
