Full Job Description

Position Summary

The Nurse Practitioner or Physician Assistant serves as a primary clinical provider at Total Primary Care, an established independent practice in Imlay City, Michigan. Working alongside the owner-physician and a second mid-level provider, this clinician delivers care across primary care, urgent care, and occupational health service lines, with primary emphasis on building longitudinal primary care relationships, managing chronic disease, and performing structured preventive care including Medicare Annual Wellness Visits.

The role offers significant clinical autonomy within Michigan scope of practice with collaborative physician oversight, a diverse patient panel, and the opportunity to be a key clinical leader in a growing practice.

Key Clinical Responsibilities

Primary Care (approximately 60-65% of clinical time)

Provide comprehensive primary care to a diverse patient panel including adults, women's health, and basic pediatrics
Perform comprehensive Medicare Annual Wellness Visits with full stacking of preventive screenings (depression screening, alcohol misuse screening, social determinants, advance care planning, obesity counseling where indicated)
Manage chronic disease including hypertension, diabetes, hyperlipidemia, COPD, asthma, depression, and other common chronic conditions
Enroll eligible patients in the practice's Chronic Care Management program and partner with the care coordination team on monthly care plans
Perform annual physicals, well-woman visits, sports physicals, and school physicals
Order, interpret, and follow up on diagnostic testing including lab work and imaging
Develop and adjust individualized treatment plans including pharmacologic and lifestyle interventions
Provide patient education on disease management, prevention, and health promotion

Urgent Care (approximately 20-25% of clinical time)

Evaluate and manage acute illness and injury including respiratory infections, UTIs, skin infections, minor lacerations, sprains, and other common acute presentations
Perform minor procedures including laceration repair, lesion excision, cerumen removal, joint injections, and incision and drainage
Triage acuity and refer to emergency care when appropriate

Occupational Health (approximately 10-15% of clinical time)

Perform DOT physical examinations once NRCME-certified (certification supported and funded by the practice within first 90 days)
Perform drug screen collections and workers' compensation evaluations
Support employer-contracted occupational health services as the program expands

Telehealth

Provide telehealth visits for established patients where clinically appropriate

Documentation and Workflow

Document patient encounters in Tebra EHR within 24 hours of visit
Use structured templates for AWV stacking, CCM enrollment, DOT physicals, and chronic care management
Code visits accurately to appropriate E&M level supported by documentation
Coordinate with the operations team, billing partner (MedCare), and care coordination partner (Prexnova) on workflow and revenue cycle

Collaboration and Practice Citizenship

Work collaboratively with Dr. Akhtar (owner-physician) and the second mid-level provider on patient care, coverage, and clinical decisions
Partner with the medical assistant, front desk team, and operations leadership on patient flow and patient experience
Participate in weekly team meetings and clinical quality discussions
Adhere to practice standards for patient experience including warm patient interaction, timely note completion, and family-conversion conversation at every appropriate visit
Maintain compliance with HIPAA, OSHA, controlled substance protocols, and Michigan scope-of-practice regulations

Required Qualifications

Active and unrestricted Michigan license as a Nurse Practitioner (FNP, AGNP, or FNP-BC) or Physician Assistant (PA-C)
Board certification through ANCC, AANP, or NCCPA
Active DEA registration
BLS certification current (ACLS preferred)
Minimum 1 year of post-licensure clinical experience in primary care, family medicine, internal medicine, or urgent care (strong new graduates with relevant rotations also considered)
Demonstrated clinical competence across the primary care scope
Willingness to obtain NRCME certification for DOT physical examinations within 90 days of hire (practice covers cost)
Comfortable with EHR documentation and standard practice technology
Excellent clinical judgment and communication skills

Preferred Qualifications

Existing NRCME certification for DOT physical examinations
Experience performing Medicare Annual Wellness Visits with stacked screenings
Experience with Chronic Care Management program enrollment and oversight
Tebra EHR experience
Occupational health or workers' compensation evaluation experience
Spanish language proficiency
Experience in a small, independent practice environment

Compensation and Benefits

Base salary $95,000 - $125,000 commensurate with credentials, experience, and prior production
Production bonus structure based on RVUs above quarterly threshold, paid quarterly

Practice Environment

Total Primary Care is an established 3,000-patient independent practice serving Imlay City and the M-53 corridor. The practice serves Medicare patients, commercial insurance patients, occupational health patients, and the regional trucking community. The team includes the owner-physician (Dr. Jabir Akhtar), one additional mid-level provider, a medical assistant/office anchor, and two front desk staff. The practice partners with Prexnova for chronic care management and care coordination, and is currently expanding its primary care identity, occupational health program, and patient retention infrastructure under a dedicated operations team.
