Future Care Manager Opportunity (RN or LPN)Geriatric Care Management

This posting is intended to build relationships with nurses interested in future opportunities and lifestyle changes, rather than candidates seeking immediate employment.

Starting Pay: $28-$30 per hour based on experience and qualifications

Imagine having the time to truly know your clients, advocate for what matters most to them, and help older adults preserve independence while working alongside a supportive interdisciplinary team.

Care Management by Natalie (CMN) anticipates adding a Care Manager to our team within the next six months. While we do not have an immediate opening, we are intentionally building relationships with nurses who may be considering a lifestyle change and are looking for meaningful, relationship based work.

This opportunity is ideal for nurses who enjoy flexible schedules, problem solving, building relationships, and guiding older adults and families through complex situations with compassion and clarity.

Who We Are

Care Management by Natalie is a private geriatric care management practice serving older adults and their families throughout Washtenaw County.

Our interdisciplinary team includes nurses, aging specialists, social workers, and dietitians who focus on the whole individual.

We believe that aging can be navigated with greater confidence, connection, and purpose when people have the right support and advocacy.

Our mission is to advocate for older adults in all aspects of their lives while helping preserve independence, dignity, and quality of life.

About the Role

Our Care Managers are advocates, coordinators, educators, and problem solvers.

This is a non clinical and non hands on role. Nursing education and experience are highly valued, but you will not be practicing bedside nursing.

You will work closely with older adults and their families to:

Conduct comprehensive assessments and develop individualized care plans
Coordinate communication between physicians, families, and service providers
Attend medical appointments and advocate for clients
Navigate hospitalizations, new diagnoses, and care transitions
Connect families with community resources and support services
Guide families through long term planning and future care decisions
Support clients through changing needs and complex situations
Collaborate closely with our interdisciplinary team
Document assessments, communication, and client updates accurately and promptly

Training and Timing Expectations

We are intentionally building relationships with nurses in anticipation of a future opening within the next six months. This opportunity is ideal for someone who is thoughtfully considering their next chapter and is not seeking an immediate employment change.

Because our work is highly individualized and relationship based, we anticipate a four to six week onboarding and training period with a gradual increase in hours and responsibilities before moving toward a full time caseload.

The ideal candidate ultimately desires full time employment and understands that meaningful care management takes time to learn and build.

Qualifications

RN or LPN education and licensure required. License does not need to be current. This role does not operate under a nursing license.
Bachelor's degree in Nursing or a related healthcare or human services field preferred.
Minimum of three years of experience working with older adults in care coordination, case management, home care, hospice, hospital, or related settings.
Strong verbal and written communication skills with high emotional intelligence and sound judgment.
Ability to work independently while collaborating within an interdisciplinary team.
Comfort navigating complex family dynamics and healthcare systems.
Proficiency with Google Workspace and electronic documentation systems. ERSP experience is a plus.
Valid driver's license and reliable transportation.
Growth oriented mindset with openness to learning, feedback, and evolving processes.
