Full Job Description

POSITION SUMMARY

The Service Coordinator provides case management for John Graham Housing & Services (JGHS) clients, including individuals and families in the John Graham Shelter in Vergennes, at JGHS affordable housing sites, and other Addison County partner sites.

MAJOR FUNCTIONS

1. Carry a caseload of 15-20 households

2. Provide clients with screening, assessment, planning, referral, and advocacy

3. Enter intake, screenings, direct service, and case note data in Community CareLink (CCL)

4. Empower people to gain access to mental health, SUD, and health care services, when needed

5. Refer to services for childcare, employment, transportation, and education, when needed

6. Attend appointments with clients when support and advocacy are needed

7. Provide support to interns and other direct service staff at JGHS sites

8. Model positive, constructive behavior for staff and clients, utilizing assertive engagement

9. Assist Shelter guests in housing navigation, including:

a. Completion and submission of Coordinated Entry Assessment for Addison County

b. Completion and submission of applications for rental properties and housing vouchers

c. Track application status and follow up

d. Completion and submission of annual recertification paperwork

e. Document case notes and retain all housing navigation paperwork in CCL

10. Participate in Housing Solutions and other collaborative, community-based meetings as assigned

11. Attend bi-weekly trainings scheduled by the Program Director

12. Meet for at least an hour each week with each household in caseload

13. Attend one-on-one supervision with the Program Director on a bi-weekly basis to discuss client progress, review files and case notes, and to consider approaches

14. Attend weekly staff meetings

QUALIFICATIONS

Bachelor’s degree in related field or equivalent experience preferred, master’s degree preferred
Ability to communicate effectively with diverse clients, partners and community members
Knowledge of service coordination, peer support, and crisis intervention
Familiarity with trauma-informed care, assertive engagement and motivational interviewing
The ability to exercise compassion and professional boundaries in a congregate setting
Working knowledge of local resources and service providers
Excellent written and verbal communication skills
Technical proficiency with electronic documentation
Good communication skills and a positive attitude
Ability to work both independently and as a team member

WORKING CONDITIONS

Location: John Graham Shelter in Vergennes, other JGHS sites, and in the community
Hours: 8:30 am – 4:30 pm Monday – Friday; work on-call on a rotating basis; some evening, weekend, and holiday hours may be assigned
Compensation: Starting wage $26-$29/hour; benefit package includes 100% premium paid on individual health insurance, 3% match on retirement plan contribution, and a generous CTO package; any hours over 40 hours/week are paid at overtime pay rate of time and a half
