Job Summary
The Social Work Case Manager plays a vital role in supporting individuals and families by providing comprehensive case management services. This position involves working closely with clients to assess their needs, develop tailored service plans, and connect them with appropriate resources. The ideal candidate will have a strong background in social work, particularly in child welfare and experience working with children and individuals with developmental disabilities.

Duties

Conduct assessments to determine the needs of clients and their families.
Develop, implement, and monitor individualized service plans that address client goals.
Provide support and advocacy for clients navigating child protective services and adoption processes.
Collaborate with multidisciplinary teams, including healthcare providers, educators, and community organizations.
Maintain accurate case documentation and ensure compliance with all regulatory requirements.
Facilitate access to resources such as childcare, counseling services, and educational programs.
Provide crisis intervention and support to clients in challenging situations.
Engage in ongoing professional development to stay current with best practices in social work.

Requirements

Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
Experience working within child welfare systems or child protective services is highly desirable.
Familiarity with issues related to developmental disabilities and adoption processes.
Strong interpersonal skills with the ability to build rapport with diverse populations.
Excellent organizational skills and attention to detail for case documentation.
Ability to work independently as well as part of a team in a fast-paced environment.
