Job Overview

Full-Time Licensed Practical Nurse (LPN)

Peaceful Purpose Hospice

Position Summary

Peaceful Purpose Hospice is seeking a compassionate, dependable, and patient-focused Licensed Practical Nurse (LPN) to join our hospice team. The LPN provides direct patient care, ongoing assessments, symptom management, caregiver education, and support to patients facing life-limiting illnesses. The LPN works under the supervision of the Registered Nurse Case Manager and in collaboration with the Hospice Interdisciplinary Team (IDT) to ensure quality end-of-life care that promotes comfort, dignity, and quality of life.

This is a full-time position serving patients in homes, assisted living facilities, nursing facilities, and other residential settings throughout the service area.

Essential Duties and ResponsibilitiesPatient Care

Perform routine skilled nursing visits as assigned.
Complete patient assessments and report findings, changes in condition, and concerns to the RN Case Manager.
Monitor and document pain, symptoms, disease progression, and response to interventions.
Administer medications and treatments as ordered and within the LPN scope of practice.
Perform wound care and other skilled nursing procedures as directed by physician orders and hospice protocols.
Reinforce hospice philosophy and goals of care with patients and families.
Assist with identifying patient and caregiver needs and communicate recommendations to the RN Case Manager.

Caregiver Education

Educate patients, families, and caregivers regarding:
Disease progression
Symptom management
Medication administration
Infection prevention
Fall prevention
Skin integrity and pressure injury prevention
Aspiration precautions
Hospice services and resources

Documentation

Complete accurate and timely documentation in the electronic medical record (EMR).
Document all patient visits, assessments, interventions, and caregiver education.
Communicate significant findings to the RN Case Manager by the end of the workday or at the beginning of the next business day if after-hours.
Ensure documentation meets regulatory, Medicare, and agency standards.

Team Collaboration

Participate in interdisciplinary team meetings as requested.
Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and other team members.
Assist in identifying changes in patient needs and contribute to care planning.

On-Call Support

Participate in after-hours and weekend visit coverage as assigned.
Respond to patient care needs when dispatched by the on-call Registered Nurse.
Provide skilled nursing visits and communicate findings promptly to the supervising RN.
Complete all required documentation following after-hours visits.

QualificationsRequired

Current Georgia Licensed Practical Nurse (LPN) license in good standing.
Minimum of one (1) year nursing experience.
Current CPR certification.
Valid driver's license.
Reliable transportation and current automobile insurance.
Ability to travel throughout the assigned service area.
Strong communication, assessment, and organizational skills.

Preferred

Hospice experience.
Home health experience.
Long-term care, oncology, geriatrics, palliative care, or rehabilitation experience.
Experience working independently in community-based settings.

Physical Requirements

Ability to stand, walk, bend, squat, kneel, and assist patients as needed.
Ability to lift up to 25 pounds.
Ability to safely enter and exit patient homes and facilities.
Ability to travel throughout the service area daily.

Schedule

Full-Time
Monday through Friday
Occasional evenings, weekends, and holiday coverage as assigned
Participation in on-call visit rotation may be required
