Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our dynamic team at a leading skilled nursing facility. In this vital role, you will provide high-quality patient care, support recovery processes, and promote overall well-being for residents with diverse healthcare needs. Your expertise will help ensure a safe, comfortable, and nurturing environment where residents receive personalized attention and professional nursing services. This position offers an exciting opportunity to make a meaningful difference in the lives of seniors and individuals requiring long-term or post-acute care.

Duties

Administer medications, including injections and IV infusions, following physician orders and established protocols while ensuring patient safety and comfort.
Conduct comprehensive patient assessments, vital sign monitoring, and physical examinations to evaluate health status accurately.
Assist with complex procedures such as catheterization, sterile dressing changes, tube feeding, and wound suturing to support recovery and comfort.
Maintain detailed medical documentation within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
Collaborate with interdisciplinary teams to develop and implement personalized care plans tailored to each resident’s needs, including dementia care, Alzheimer's care, and memory care.
Provide triage services by assessing urgent health concerns, prioritizing care needs, and coordinating timely interventions in accordance with emergency protocols.
Support discharge planning, patient education, and health coaching to empower residents in managing their health effectively.
Follow infection control procedures diligently to prevent the spread of infectious diseases within the facility.
Ensure accurate medication administration using ICD coding standards such as ICD-10 or ICD-9 when documenting diagnoses.

Experience

Valid state licensure as a Registered Nurse (RN) with current certification required.
Prior experience working in skilled nursing facilities or long-term care environments preferred; experience in hospital or ICU settings is highly valued.
Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and patient management.
Experience administering injections, IV therapy, and managing acute pain or complex medical conditions including ventilator management or hospice care is advantageous.
Knowledge of geriatrics, dementia care—including Alzheimer's—and behavioral health considerations essential for providing compassionate senior care.
Familiarity with medical terminology, ICD coding (ICD-10/ICD-9), and diagnostic evaluation techniques necessary for accurate recordkeeping and case management.
Ability to perform physical examinations, specimen collection/processing, phlebotomy, and basic life support (BLS) certification required.
Strong understanding of infection control practices, anatomy knowledge, physiology principles, and airway management techniques critical for patient safety.
