As a Registered Nurse MDS Coordinator (RN/LPN) at Bayshore Nursing & Rehab, you will play a vital role in ensuring the seamless coordination of services for our residents. Your expertise in nursing care and RAI/MDS practices will enable you to effectively assess, plan, and document resident care, while collaborating with our interdisciplinary team to deliver high-quality care. [Job Title]: Registered Nurse MDS Coordinator (RN/LPN) is responsible for:

Coordinating the RAI/MDS process, ensuring accurate and timely completion of assessments and care plans
Collaborating with the interdisciplinary team to identify and address resident needs, goals, and preferences
Developing and implementing individualized care plans, incorporating resident-centered care and evidence-based practices
Maintaining accurate and up-to-date resident records, including care plans, progress notes, and incident reports
Participating in quality improvement initiatives, identifying areas for improvement, and contributing to the development of solutions
Providing education and support to nursing staff, family members, and facility staff on RAI/MDS processes and best practices
