Full Job Description

Job Title: Housing Navigator

Report to: Shelter Supportive Services Operations Director

Classification: Non-Exempt, 40 hours per week

SUMMARY

The Housing Navigator works in conjunction with the Shelter Supportive Services Operations Director who acts as the Housing Case Manager for families with children under the age of 18 enrolled in the Wisconsin Housing Support Services Program (WHSSP). The Navigator is responsible for managing referrals to the program, assessing families’ eligibility, providing housing consultation and transition supports to assist families with children to achieve housing as soon as possible.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

· Manage all referrals for WHSSP from HALO and other agencies/organizations, and screen families for eligibility.

· Conduct a housing consultation meeting with all referrals to determine final eligibility, and:

o Assess preferences for, and all barriers to housing, including affordability and accessibility.

o Also assess mainstream benefit needs and make referrals/linkage to community service providers for health, income, employment, transportation, child care, etc.

o Ensure client has been entered into the Coordinated Entry system or enter client if deemed appropriate.

o Inform the Shelter Supportive Services Operations Director (Housing Case Manager) of any assistance needed with obtaining identification documents necessary to apply for housing.

· Create a housing support plan based on the assessment.

· Provide transition supports for the family to prepare for and move to housing:

o Assistance with housing search within HUD Fair Market Rent limitations.

o Assistance with credit issues related to applying

o Assistance with applying for the housing and available housing assistance options

o Education and assistance on signing the lease and responsibilities.

o Conducting an inspection and ensuring the unit meets Housing Quality Standards.

o Assist family with applying for mainstream benefits and accessing all needed community resources from assessment.

o Inform the Director of Supportive Services of any financial assistance needed with the move (security deposit, utilities set up, etc.)

o Work with Feather a Nest to obtain furniture as needed for the family.

· Transition the family to the Director of Supportive Services (Housing Case Manager) for services related to achieving housing stability.

· Provides data and reports as requested.

· Provides outreach and information about the program to outside referral sources.

· Attends staff meetings as required.

· Other duties as assigned

· Agency policies, procedures and established practices are followed.

· Independently assume position functions and integrate knowledge into daily performance.

Qualifications

· Bachelor’s degree in social work, human services, or related field required.

· One-year experience providing homelessness and housing accessibility services required.

· Ability to assess the needs of service recipients and knowledge of community and mainstream resources to meet those needs.

· Experience with Coordinated Entry and HMIS systems, or ability to become trained.

· Experience with using Motivational Interviewing, Cultural Competency, and Trauma Informed Care models or ability to become trained.

Knowledge, Skills & Abilities

· Must have own transportation and a valid driver’s license.

· Possess the necessary written and oral skills to communicate clearly and effectively with staff, clients, and the general public.

· Able to communicate with a diverse population

· Skilled at using Microsoft Office products.

· Exceptional judgement and ability to function and multi-task under time constraints.

Work environment/physical demands

· Must be able to bend, stoop, stretch, and climb stairs.

· Frequent local travel.
