Full Job Description

Position Title: Coordinated Entry Specialist

Supervisor: Director of Homelessness Services

The Coordinated Entry Specialist is responsible for facilitating initial housing and supportive service connections

for individuals and families experiencing homelessness. This role manages the Coordinated Entry process, ensuring

clients are connected to available resources and services that support their path toward permanent housing and

long-term stability.

The Coordinated Entry Specialist serves as the primary point of contact for the intake and initial processing of

individuals seeking homeless services within the Waukesha County Local Homeless Coalition. Responsibilities

include client intake, prevention and diversion services, intervention, housing navigation, service coordination, and

referrals.

This position assists clients in moving smoothly through the Coordinated Entry process and collaborates with case

managers and service providers to ensure access to appropriate housing and supportive services as clients work

toward self-sufficiency.

The Coordinated Entry Specialist focuses on helping clients navigate the Coordinated Entry system, coordinating

service delivery, identifying system improvements, and helping expedite access to housing resources.

Key areas of focus include:

Providing outreach services that engage individuals who may be unlikely to seek assistance on their own.
Assessing the needs of individuals and families experiencing homelessness.
Providing case management services that build trust, engagement, and ongoing support.
Facilitating access to housing and supportive services that lead to safe, stable, and permanent housing as

quickly as possible.

Responsibilities:

The Local Coordinated Entry Lead serves as the primary liaison between the Wisconsin Balance of State

Continuum of Care (WI BOSCOC) and participating agencies within the Local Coordinated Entry System.

Responsibilities include, but are not limited to:

Serve as the Coordinated Entry subject matter expert for the Local Coordinated Entry System.
Ensure accurate and consistent communication between WI BOSCOC, the Local Coordinated Entry

System, and local homeless coalitions.

Generate and analyze reports within the Homeless Management Information System (HMIS).
Ensure participating agency staff complete all required Coordinated Entry trainings.
Attend all required Coordinated Entry and Coordinated Entry Lead trainings.
Communicate Coordinated Entry updates, policy changes, and system enhancements to local homeless

coalition partners.

Collaborate closely with the Non-HMIS List Holder.
Maintain all Agency Agreements and Staff Agreements associated with the LCES.
Update and maintain the WI BOSCOC training spreadsheet for participating agencies.
Monitor compliance with WI BOSCOC Coordinated Entry policies and procedures.
Ensure required marketing and outreach activities are conducted throughout the LCES.
Maintain an after-hours emergency access plan and ensure all relevant stakeholders are informed of

available emergency service procedures.

Ensure compliance with all HMIS policies, procedures, and data standards.
Perform the duties of the Coordinated Entry Non-HMIS List Holder.Hebron Housing Services

Position Description

Non-HMIS List Holder Duties

Maintain the Non-HMIS Prioritization List and supporting Google Forms, including troubleshooting issues

and communicating concerns as needed.

Upon request from housing programs, provide prioritization information for the highest-priority eligible

participant, including:

o Unique identifier

o Assessment information (VI-SPDAT score)

o Length of time homeless

o Chronic homelessness status

o Referring agency contact information

Respond promptly to requests from participating agencies and staff.
Attend all required Coordinated Entry and List Holder trainings.
Work collaboratively with the Local Coordinated Entry Lead and participating agencies.

Qualifications:

Associate degree (2-year degree) in Social Work, Human Services, or a related field required; Bachelor's

degree preferred.

Equivalent combinations of education and relevant work experience may be considered.
Strong technical proficiency, including Microsoft 365 applications.
Knowledge of reporting processes and data analysis.
Excellent organizational and time management skills.
Strong problem-solving and critical-thinking abilities.
Knowledge of consumer life skills, employment readiness, housing stability, home management, health,

and wellness resources.

Effective written and verbal communication skills.
Ability to work collaboratively in a team-oriented environment and perform effectively under pressure.
Ability to lift and carry 30-50 pounds.
Ability to sit and/or stand for extended periods of time.

Schedule:

This is a full-time, hourly, non-exempt position requiring a 40-hour work week. Various shifts may be available,

including first, second, and third shift schedules. Occasional evening, weekend, and holiday hours may be required

to meet program and client needs.

Pay: $22.00 - $24.00 per hour
