Job Title: Hospice RN Care Transitions Liaison | Admissions & Referral Partnerships

Schedule: Monday-Friday
Compensation: $90,000-$130,000 DOE + Mileage Reimbursement + Comprehensive Benefits Package

The right hospice conversation, at the right time, can change everything.

As a Hospice RN Care Transitions Liaison, you'll help patients and families understand their options, guide them through the hospice journey, and ensure they have access to compassionate care when they need it most. You'll also build trusted relationships with hospitals, providers, senior living communities, and referral partners throughout the Twin Cities.

What You'll DoCare Transitions & Admissions

Meet with patients and families to discuss hospice services and eligibility.
Conduct hospice consultations and clinical assessments.
Facilitate timely hospice admissions and seamless transitions of care.
Educate patients and families on hospice philosophy, services, and care options.
Partner closely with the clinical team to ensure exceptional patient experiences.

Referral & Community Partnerships

Build and maintain relationships with hospitals, physicians, case managers, discharge planners, senior living communities, and referral partners.
Serve as a trusted hospice resource throughout the Greater Twin Cities healthcare community.
Educate referral sources on hospice eligibility and available services.
Participate in outreach efforts that strengthen existing partnerships and create new opportunities.

Patient Advocacy

Help patients and families navigate complex healthcare decisions.
Identify patients who may benefit from hospice services.
Provide compassionate education, guidance, and support throughout the decision-making process.
Help ensure patients receive timely access to high-quality end-of-life care.

What We're Looking For

Active Minnesota Registered Nurse (RN) license.
Hospice experience preferred.
Strong consideration given to nurses with backgrounds in ICU, ER, Oncology, Palliative Care, Home Health, Case Management, Care Coordination, Discharge Planning, or other high-acuity clinical settings.
Outstanding communication and relationship-building skills.
Strong clinical judgment and professionalism.
Ability to work independently while managing a field-based schedule.
Valid driver's license and willingness to travel throughout the Greater Twin Cities Metro Area.
