The Supervising Professional is responsible for setting up and managing direct care services for participants who qualify through assessment. They develop the participant service plan with input from the Participant Representative (PR) and assessment findings and may assist with competency training as needed. The Supervising Professional serves as a resource to participants, PRs, and workers regarding program use and services that support participant health and safety in the community. Responsibilities may also include training, supervision, service evaluation, quality assurance, and staying current on program policies and changes. They may assist with home visits, competency visits, committees, and meetings related to client services.
THE ROLE
As an Supervising Professional, you will have the following responsibilities:
- Conduct home visits, assess participant needs, and develop service delivery plans in compliance with DHS guidelines.
- Monitor services and maintain accurate, timely, and legible documentation according to agency standards.
- Manage assigned caseloads by completing required visits and routinely reviewing weekly and monthly documentation to ensure compliance and productivity standards are met.
- Work effectively with individuals from diverse backgrounds in various home and community settings, including travel throughout metro and rural areas of Minnesota.
- Maintain knowledge of common medical diagnoses and appropriate care needs for participants with varying conditions.
- Communicate professionally and effectively with participants, Participant Representatives (PRs), workers, referral sources, and interdisciplinary team members to support quality care.
- Provide participants and PRs with information on available resources and services.
- Maintain current knowledge of DHS regulations and agency policies related to Community First Services and Supports (CFSS).
- Assist with worker supervision and support as requested.
- Provide training and guidance to workers as needed, and communicate participant care needs with participants, PRs, and staff.
- Participate in agency and community trainings at the request of their supervisor.
- Promote participant and workplace safety by maintaining a safe and supportive environment.
- Participate in ongoing professional development and identify opportunities for continued growth.
- Demonstrate flexibility and adaptability in response to changing needs and priorities.
- Support the mission of Hmong Home Health Care by following agency policies and procedures, using resources responsibly, and completing additional assigned duties.
- For Licensed Social Workers only: Maintain an active Social Work license and complete required continuing education for licensure renewal.
QUALIFICATIONS
A Supervising Professional must be qualified in one of the following ways:
- A bachelor degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older.
- An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older.
- A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older.
- A minimum of 50 hours of education and training related to human services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in the three bullet points above.
- Willingness to learn and comply with home health care regulations, requirements, and protocols.
- Strong critical thinking, creativity, and problem-solving abilities.
- Ability to lead positively and adapt to change effectively.
- Demonstrated professionalism and commitment to quality service.
- Excellent organizational, communication, interpersonal, and problem-solving skills.
- Interest in learning federal and Minnesota state regulations related to services for individuals with disabilities and older adults.
- Ability to understand and work within internal systems that support service delivery.
- Must successfully pass a Minnesota Department of Human Services background study.
- Must possess a valid driver’s license, maintain a clean driving record, and have reliable transportation for work-related travel.
- Ability to speak conversational-level Hmong, Vietnamese, Laotian, or Karen highly preferred.
ADDITIONAL INFORMATION
We are hiring for this position immediately and hope to have final candidates selected in two to four weeks.
Work Hours: This exempt position is a 90% remote position, working from home or making visits at client's home in the community. Business hours are 9am - 5pm; Monday-Friday. Hmong Home Health Care is a highly collaborative and customer-focused work environment. The CFSS team is part of this collaborative culture via Teams , bi-weekly in-person team meetings, team building opportunities, and all-company events.

The salary for this role represents a pay range. Please note that awarded wages are commensurate with a candidate's experience.
