Full Job Description

AME Community Services, Inc. is a highly respected organization known nationally and internationally for its advanced, person-centered supports for individuals diagnosed with Prader-Willi Syndrome (PWS). AME provides exceptional residential and community-based services to people with developmental disabilities, specializing in complex behavioral, medical, and support needs.

We are seeking a Program Coordinator (PC) to provide leadership at one of our Community Residential Settings in Golden Valley, Minnesota. This position supports three adults with Prader-Willi Syndrome.

The Program Coordinator is a front-line manager responsible for the daily operations of the home and the overall quality of services provided. The PC works closely with the Program Director to supervise Direct Support Professionals (DSPs), ensure compliance with Minnesota 245D regulations, oversee program planning, and provide direct care during scheduled shifts. The PC serves as the liaison between the interdisciplinary team and the agency and must be available in the event of emergencies.

Program Coordinator Job Description

Direct Support

Serves as a model in effectively providing all levels of direct support and assistance.
Communicates effectively with clients through active listening, appropriate responses, and follow-up on concerns.
Assists in planning and ensuring active participation in community events and activities.
Supports clients in maintaining and developing friendships and family relationships.
Identifies necessary resources and advocates for client needs with the Program and/or Executive Director.
Promotes the health, safety, and welfare of clients and ensures the physical environment meets their needs and preferences.
Maintains a current driver’s license and safe driving record; transports residents as necessary.
Continuously evaluates the quality of support provided and strives for improvement.

Program Management and Evaluation

Develops, implements, documents, and evaluates client outcomes, supports, and methods based on measurable and observable criteria.
Ensures positive support strategies and person-centered principles are incorporated into service plans.
Facilitates team meetings and serves as an effective team leader.
Completes required documentation and reports regarding progress toward client goals.
Ensures medical appointments are scheduled and attended; monitors health data and communicates follow-up needs to staff and the extended support team.
Informs the Program Director and ensures required reports are completed in the event of incidents or emergency use of manual restraints.
Provides ongoing direction, supervision, and support to DSP staff to ensure consistent implementation of supports and accurate documentation.
Implements and promotes policies and procedures related to regulatory agencies, including Minnesota 245D compliance.
Ensures quarterly fire drills are conducted and documented.
Maintains accurate and confidential documentation in accordance with data privacy policies.

Personnel Management and Staff Relations

Demonstrates leadership that promotes positive and professional relationships among staff, families, and other providers.
Assists the Program Director with recruitment and hiring.
Assists in completing staff performance evaluations.
Provides ongoing coaching and performance feedback; documents concerns and assists with disciplinary action when necessary.
Communicates effectively and regularly with supervisors and peers.
Prioritizes tasks and responsibilities to ensure deadlines are met.

Training and Staff Development

Attends recommended in-service training and continuing education opportunities.
Participates in management meetings.
Coordinates and facilitates staff meetings and ensures follow-up.
Assists in coordinating and scheduling new employee orientation and ongoing in-service training; ensures documentation is completed.
Develops staff schedules in a timely manner and ensures time-off requests are processed appropriately.
Ensures adequately trained staff are scheduled to meet client needs and maintain high-quality care standards.

Pay: $23.00 - $25.00 per hour
