Full Job Description

About the Role

As a Clinic Nurse Case Manager, you will play a vital role in coordinating holistic care for PACE participants. You’ll collaborate closely with an interdisciplinary team to ensure each participant receives high-quality, person-centered support. This position is perfect for an RN who excels in communication, critical thinking, and nurturing long-term clinical relationships.

What You’ll Do

Serve as the primary nursing point of contact for assigned participants.
Complete assessments, develop individualized care plans, and monitor participant needs.
Provide clinical oversight and collaborate with providers, therapists, social workers, and other team members.
Coordinate care across clinic, home, and community settings.
Document clinical findings and maintain accurate, timely records.
Participate in daily team meetings and support overall PACE operations.
Promote participant independence, safety, and quality of life.

What We’re Looking For

At least 1–2 years of nursing experience, preferably in geriatrics, clinic, case management, home health, long-term care, or similar settings.
Strong assessment, communication, and teamwork skills.
Ability to manage multiple priorities and adapt in a dynamic care environment.
A passion for serving older adults and supporting a mission-driven program.

Employment Specifications:

Education/Experience: Degree in nursing from an accredited college or university. At least two years’ as staff nurse, preferred. One year’s experience with frail or elderly population.

Licensure, registry or certification required: Current unencumbered North Dakota Nursing License.
