Full Job Description

POSITION: Case Manager
STATUS: Nonexempt
LOCATION: Jane’s House
IMMEDIATE SUPERVISOR: Shelter Manager JH
HOURS: Full-time, 40 hours per week (Monday–Friday, 8:00 a.m.- 5:00 p.m., with one 10:00 a.m.–7:00 p.m. weekday shift. Occasional evening and weekend work required.

SUMMARY: Provides survivor-centered, trauma-informed case management, advocacy, and coordination of services to adult and child residents of the agency’s emergency and transitional shelter programs. The Case Manager partners with clients to identify strengths, assess needs and barriers, develop individualized service plans, and connect clients with resources that promote safety, healing, housing stability, self-sufficiency, and long-term independence.

Works in close partnership with the Human Trafficking Case Manager to ensure consistent and seamless case management services for all shelter residents and collaborates with agency staff, multidisciplinary teams, and community partners to coordinate services and remove barriers while respecting each client’s autonomy, dignity, self-determination, and informed choice.

POSITION REQUIREMENTS

Essential position requirements include, but are not limited to:

Minimum of a bachelor’s degree in social work, psychology, or a related field. Two (2) years of related experience preferred.

Demonstrated understanding of trauma-informed, survivor-centered, and culturally responsive practices.

Ability to establish trusting and professional relationships with individuals from diverse backgrounds.

Ability to maintain appropriate professional boundaries while exercising sound judgment and discretion.

Demonstrated initiative, sound judgment, and the ability to work independently while appropriately seeking guidance and collaboration when needed.

Excellent verbal, written, organizational, and interpersonal communication skills.

Ability to prioritize responsibilities, manage multiple tasks, and adapt to changing client and program needs.

Ability to work collaboratively as part of a multidisciplinary team.

Commitment to the agency’s mission, values, policies, and procedures.

Must have a reliable vehicle and meet agency driving policy requirements, including a valid driver’s license and proof of required automobile insurance. Local travel is required.

Ability to obtain a Class D driver’s license as required.

General physical requirements include, but are not limited to, bending, stooping, climbing stairs, prolonged sitting of more than four (4) hours, and lifting or carrying up to 40 pounds.

ESSENTIAL DUTIES INCLUDE BUT ARE NOT LIMITED TO:

Advocacy and Case Management

Manage an assigned caseload of approximately 20–25 shelter residents, providing individualized case management and adjusting caseload responsibilities as needed to meet client, program, and agency needs.
Conduct initial and ongoing assessments to identify client strengths, needs, barriers, safety concerns, resources, and priorities using survivor-centered and trauma-informed practices.
Partner with clients to develop individualized safety plans and service plans that reflect their identified goals, priorities, and choices.
Provide advocacy, crisis intervention, problem-solving support, and case management services that promote client safety, self-determination, healing, stability, and independence.
Collaborate and advocate with community partners, social service agencies, and other service providers to connect clients with needed services, resources, and supports and address barriers to access.
Assist clients in identifying and addressing barriers that may impact their ability to achieve housing and other self-identified goals.
Support clients in exploring housing options and provide recommendations to clinical staff regarding appropriate housing programs and resources.
Assist shelter residents who need relocation outside of the agency’s service area.
Provide client transportation to community locations as needed and in accordance with agency policy.
Facilitate a weekly life skills group or other educational and supportive programming as assigned.
Conduct morning meetings with shelter residents as assigned to identify needs, address barriers, share relevant program information, and support progress toward client-identified goals.
Assist clients with discharge and transition planning by coordinating appropriate referrals, resources, supports, and community connections that promote continuity of services, safety, and stability.
Serve as the primary point of contact for the Safe Haven for Pets Program, coordinating confidential temporary shelter and related services for clients’ pets with participating veterinary providers.

Collaboration and Service Coordination

Work in close coordination with the Human Trafficking Case Manager to ensure seamless case management services for all shelter residents, coordinating caseload assignments and adjusting responsibilities as needed to respond to changing client and program needs.
Attend and actively participate in weekly shelter staffing meetings.
Collaborate with counselors, residential staff, housing staff, and other agency personnel to support coordinated and effective services for clients.
Communicate relevant client needs, barriers, concerns, and service recommendations with appropriate team members while maintaining confidentiality and respecting client autonomy.
Participate in collaborative service planning and provide recommendations regarding housing, safety, resources, and other client needs.
Work collaboratively with landlords, medical and behavioral health providers, schools, legal providers, community organizations, and other partners as appropriate to coordinate services and reduce barriers.
Maintain respectful, professional, and trauma-informed interactions with clients, coworkers, and community partners.
Utilize approaches that emphasize safety, trust, transparency, collaboration, empowerment, choice, and respect for individual experiences and cultural differences.

AGENCY COMPLIANCE

Maintain comprehensive, accurate, timely, and organized client records utilizing the agency’s automated data and case management system.
Adhere to the agency’s confidentiality policies, maintaining a high-level standard of confidentiality.

· Compile and accurately report all applicable program statistics on time and as directed.

· Report any known or suspected child abuse to the Mississippi Department of Child Protection Services

· Report any suspicious activity or persons to the immediate supervisor or 911.

· Attend and participate in agency staff meetings and trainings.

· Report to work on time as scheduled

· Maintain a valid driver's license and proof of required automobile insurance.

Maintain appropriate professional boundaries and follow agency ethical standards when providing services to clients.

· Perform other duties or special projects as assigned by the supervisor or CEO.

OTHER

Participate in hurricane evacuation and facility preparation as needed.
During emergencies and extreme circumstances, may be required to assist with 24-hour shelter coverage.
Work collaboratively with other agency programs and staff to ensure effective communication regarding agency and client needs, concerns, recommendations, and services.
