RN/LPN MDS Coordinator (Long-Term Care)

Job Title: RN/LPN MDS Coordinator (Long-Term Care)

Job Summary:

We are seeking a highly skilled and dedicated RN/LPN MDS Coordinator (Long-Term Care) to join our team at Hope Creek Nursing and Rehabilitation Center in East Moline, Illinois. As a valued member of our care team, you will play a critical role in ensuring the exceptional care and well-being of our residents. Responsibilities:

Coordinate and complete the Minimum Data Set (MDS) process for all residents, ensuring timely and accurate completion of assessments, care plans, and updates
Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals to develop and implement individualized care plans
Collect and analyze data, identifying trends and areas for improvement, and presenting findings to the care team
Participate in quality improvement initiatives, supporting the development and implementation of programs to enhance resident outcomes and satisfaction
Maintain accurate and detailed records, ensuring compliance with regulatory requirements and internal policies
Provide education and support to residents, families, and staff on MDS processes, care plans, and resident needs

Requirements:

Currently licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois
Minimum of [insert minimum years of experience] years of experience in long-term care or a related healthcare setting
Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
