Full Job Description

JOB SUMMARY:

The Case Manager will provide daily care coordination, case management, coaching, consultation & intervention to patients. (Patient Centered Medical Home = PCMH)

Qualifications:

1. Must be at least 18 years of age

2. Must have an unencumbered Louisiana Practical Nurse (LPN) license or a Medical Assistant certification (CMA or RMA).

3. The ability to relate with effectiveness to the public, the patient, and the health care providers.

4. The ability to effectively perform the duties that are part of the clinical activities.

5. Case management experience preferred.

Responsibilities:

1. Coordinate health care referrals by giving the referral consultant or specialist the clinical reason for the referral & pertinent clinical information. (PCMH)

2. Tracks the status of referrals, including required timing for receiving a specialist’s report. (PCMH)

3. Follow-up to obtain a specialist’s or consultant’s report.

4. Records pertinent information in medical records. Duplicates and collects printed materials.

5. Establishes & documents agreements with specialists in the medical record if co-management is needed.(PCMH)

6. Ask patients/families about self-referrals & request reports from clinicians. (PCMH)

7. Receives and routes phone calls to appropriate personnel and take accurate and complete messages. Responds to requests for assistance and/or general information. Greets visitors and provides requested information. Assure clinic preparation and follow-up of all programs.

8. Collects information for statistical reports. Reviews records for accuracy and completeness.

9. All other duties as specified in company's job description.
