We provide individual health services for people with any type of behavioral health concerns such as substance abuse, emotional issues, and intellectual and developmental disabilities.

DESCRIPTION

Under the general supervision of the Program’s Supervisor, the Client Services Manager (CSM) assesses the needs of the consumer and the consumer’s’ family, when appropriate, and arranges, coordinates, monitors, evaluates, and advocates for services to meet the specific consumer’s needs through Family / Person center planning (PCP) and the development of an individual plan of service (FPOS/IPOS).
The goal for Child and Family CSM’s is to provide programs and services that promote normal development, promote healthy family functioning, support and preserve families, reunite families who have been separated, and reduce the usage of, or shorten the length of stay in, psychiatric hospitals and other substitute care settings.
When providing Home Based services, services relating to plan goals will be provided primarily in the family home or a setting of the family’s choosing. The family unit is the focus of treatment. The service style must support and empower families, parent and professional teamwork and community collaboration with other service providers.
The Client Services Manager is responsible for addressing both the individual consumer’s biopsychosocial status as well as the state of the social system in which case management operates, assuring the implementation and delivery of services to children and their families in keeping with the principles of recovery, self-determination and resilience, as well as developing and maintaining a therapeutic relationship with the consumer.
CSM’s are also responsible for linking consumers with external systems to provide additional services, resources, and opportunities consistent with qualitative and quantitative performance goals and standards established by the department.
In addition, Master level CSM’s are assigned to provide Intensive Case Management and /or Therapeutic Interventions to their consumers.
Client Services Managers may be required to provide crisis support services, possibly to include pre-screening for inpatient or partial hospitalization, if needed for assigned consumers and families.

Duties and Competencies

Consumer Engagement

Engages the consumer by:

Face to face contact with newly assigned consumers within seven (7) business days.
Face to face contact with consumers at or above 50% of time worked.
Educating consumers regarding Person Centered Planning and self determination opportunities.
Scheduling appointment and seeing a consumer within one (1) business day for any open consumers discharged from inpatient psychiatric hospital.
Assisting family members with problems relating to consumer conditions and needs and encourages their support in the treatment area.
Educating consumers as to the Family/Person Centered processes, self-determination, recipient rights and other issues of consumer empowerment. Assists consumers with their individual physical, personal care, (re)habilitative needs, and transportation as necessary.
Case Management

Provides for case management services by:

Convening and presiding over annual Family/ Individual Plan of Service meetings for each assigned consumer.
Recording and documenting progress notes and completed SAL’s within twenty-four business hours.
Completing FPOS/IPOS at a frequency noted in the plan of service and reviewing the FPOS/IPOS’s in a timely manner; not to exceed 365 calendar days. (Completes required assessments and status reports using Person Centered processes. Consumer Care Behavior Risk Management Committee and Peer Review Committee reviews are coordinated through and integrated with this process.)

Provides a copy of the consumer’s FPOS/IPOS within 15 calendar days.
Completing case management narratives and other assessments as required by the case and in a format required by the agency. This includes CAFAS, PECFAS, Needs Assessment/ Biopsychosocial Re-Assessment/Pain Management/Health History (during FPOS/IPOS process) for each case at least annually.
Monitoring consumer progress including, as appropriate, contacts and status reviews.

Maintaining complete and detailed case records and service activity documentation consistent with MDCH standards and agency procedures.
Meets the requirements of third party payor sources assuring revenue generation at levels which meet or exceed revenue targets established for the program.
Maintaining an up-to-date master caseload list within the prescribed format established by the agency.
Provides transportation as needed in order to assist consumers and families in procurement and maintaining residential, daily living and supportive services; linking financial and medical benefits, legal and habilitation services.
Program Development

Conducts program development by:

Assessing the child to include; growth and development; family relationships and supports; ability to function in areas of school, community, family and interpersonal relationships.
Coordinating the Family/Person Centered Planning process in developing a PCP (Person Center Plan) for agency consumers assigned to the caseload.
Contacting consumer and/or guardian to assure Family/Person Centered processes including development of FPOS/IPOS meeting agenda and planned attendees are appropriate in the opinion of consumer/guardian.

Providing individual, group and/or family support services as needed, in accordance with BC/BS, Children's Model Waiver, Habilitation Waiver, Medicaid, Joint Commission and FIA rules and regulations.
Providing consultative services to CMHA staff or other Provider personnel in order to enhance consumer functioning.
Implementing interdisciplinary peer review recommendations and decisions.
Service Delivery

Utilizes a Family Person Centered Plan to implement and monitor the Family Plan of Service for each assigned consumer. This includes:

Providing follow-up and monitoring services related to consumer/guardian approval of services, FPOS/IPOS goals and objectives and planned interventions.
Convening FPOS/IPOS team meetings if status reviews indicate a significant revision of the FPOS/IPOS is warranted to continue or terminate service delivery.
Coordinating services delivered with CMHA staff and other agencies as defined in the FPOS/IPOS.
Providing services at quality levels that meet or exceed indicator thresholds established through the Utilization Review (UR) and as defined in the FPOS/IPOS.

Assisting consumers in procurement of supportive services, financial and medical benefits, legal, and habilitation services as needed.
Consulting with the nurse and/or physician about medical concerns and treatment plans.
Assisting consumers in obtaining hospital admission and facilitating alternative residential placement when therapeutically indicated.
Provide Therapeutic Interventions such as anger management, parenting skills, etc…when necessary (Master’s level clinician).
Perform clinical assessments based upon professional qualifications and licensure, utilizing program specific assessment tools and evidence based practices.
Linking and Coordinating with Community Resources

The CSM works with the consumer to:

Provide, secure and/or advocate for necessary support services to maintain and enhance consumer functioning in the family unit and community.
Procures emergency services and /or daytime crisis intervention services as necessary.
Coordinate referrals and services with other community agencies.
Provide Therapy Services (Master’s level CSM)

Provides therapeutic services when appropriate by:

Intensive case management assignment.
Performing clinical assessments based upon professional qualifications and licensure, utilizing program specific assessment tools and evidence based practices.
Providing individual therapy and group based therapeutic interventions when appropriate.
Organizational and Personal Development:

Completes all mandatory training and certifications by their due date. Maintains 24 hours of child specific training annually. Attends workshops, trainings, and engages in independent study to promote personal and professional development which will support program/departmental goals.
Complies with all standards established and evaluated under the UM/UR and Peer review process.

Fosters, promotes and demonstrates alignment with the organization’s mission, vision and goals.
Participates actively in performance improvement activities.
Assures professional, ethical behavior and relationships
Observes all requirements of law and regulation with respect to Recipient Rights and confidentiality of health care.

Other related duties as assigned:

Completes additional IPOS case management duties as assigned.
Makes presentations to the public including school, civic and community agencies and relevant meetings, as needed.
Maintain current skills and developing new skills to maintain current competence essential for implementing job duties and responsibilities.
Follows oral and written direction completing tasks as assigned by due dates.
Maintains working knowledge of software applications used in performance of daily duties.

Education, Training and Licensing Credentials:

Bachelor’s Level (Pay Grade 7)($26.35-32.94/hr)

Minimum of a Bachelor’s degree in social work is required.
Appropriate Michigan licensure/certification is required.

Master’s Level (Pay Grade 9)($31.88-39.84/hr)

Master's Degree in Social Work, Psychology, or Counseling with State of Michigan Licensure.
Master’s level such as:
- Licensed Master’s Social Work
- Limited License Psychology
- Licensed Professional Counselor
- Licensed Marriage and Family Therapist

Experience:

B.A. CSM

One (1) year supervised experience in the examination, evaluation, and treatment of minors and their families within the specific consumer population in which the position holds responsibility child with mental illness or SED (QMHP*), child with developmental disability (QMRP**) required. CAFAS training preferred.
Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served.
Demonstrated knowledge of the clinical needs, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individual’s status; interpret the appropriate information needed to identify each individual’s requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group – (child, adult and geriatric).

M.A. CSM

Two (2+) years work experience within the specific consumer population in which the position holds responsibility (child with mental illness or SED (QMHP*), child with developmental disability (CMHP*** & QMRP**), home based services (CMHP***) and CAFAS training preferred.
- - QMHP - Individual with specialized training or one year experience in treating or working with a person who has mental illness as defined under the Michigan PIPH/CMSP Provider Qualifications per Medicaid Services.
- **QIDP -Qualified Intellectual Disability Professional (QIDP) - Individual with specialized training or one year experience in treating or working with a person who has intellectual disability.
Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served.
Demonstrated knowledge of the clinical needs, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individual’s status; interpret the appropriate information needed to identify each individual’s requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group – (child, adult and geriatric).

Note: A related degree and /or a limited license may be considered in lieu of the above where provider qualifications as defined under the Michigan Mental Health Code and/or Medicaid Provider Manual and appropriate supervision exist.

Other Competencies and Abilities:

Ability to work in a complex, detail-oriented, deadline structured busy organization.
The following physical and mental abilities are required: frequent lifting or carrying up to 15 pounds, must be able to hear, frequent standing/walking going up and down stairs due to community outreach basis, ability to communicate effectively in person and/or phone, the ability to read, analyze and interpret information, ability to write reports, correspondence and procedures, ability to maintain consumer charts/records, ability to define and solve practical problems, collect data, establish facts and draw valid conclusions, ability to influence others, and the ability to conduct patient assessments and treatment.
Demonstrated proficiency in MS Outlook, MS Office, electronic medical records (Encompass II) and such other applications if/when required.
Ability to be trained and successfully complete other computer training as required.
Valid driver’s license, proof of current automobile insurance and driving record determined by the Authority’s insurance carrier to be eligible for coverage if required as part of positions duties.
